目的评价超声心动图在先天性心脏病镶嵌治疗中的应用价值。方法对7例先心病外科术后残余分流及2例室间隔缺损合并动脉导管未闭的患儿采用超声心动图引导、心导管介入的镶嵌技术封堵进行治疗;1例室间隔缺损合并肺动脉瓣狭窄患儿及1例房间隔缺损合并三尖瓣下移畸形成人患者,术中联合超声心动图评价外科矫治术的镶嵌治疗;1例超声心动图引导房间隔缺损介入封堵失败后,行外科手术的镶嵌治疗。结果 7例先心病术后残余分流的患儿分别放置一个封堵伞或弹簧圈后使残余分流消失,2例室间隔缺损合并动脉导管未闭的患儿先放置弹簧圈封堵动脉导管未闭,介入治疗后7 d成功行外科手术治疗。1例室间隔缺损合并肺动脉瓣狭窄患儿和1例房间隔缺损合并三尖瓣下移畸形患者,超声心动图评价术后即刻疗效满意。1例超声心动图引导巨大房间隔缺损介入封堵失败后,即刻行外科手术成功。结论超声心动图敏感、方便,可实时指导先心病镶嵌治疗,为手术提供重要信息,术后即刻评价治疗效果,安全可靠,在先心病镶嵌治疗中发挥重要作用。
Objective To evaluate the value of echocardiography in mosaic therapy of congenital heart disease. Methods Seven patients with residual shunt after surgery and two cases of ventricular septal defect with patent ductus arteriosus were treated with echocardiography and catheterization inlay technique. One patient with ventricular septal defect complicated with pulmonary valve Stenosis in children and one case of adult patients with atrial septal defect and tricuspid regurgitation, intraoperative and postoperative echocardiographic evaluation of surgical treatment of mosaic; 1 case of echocardiography guided atrial septal defect after the failure of interventional surgery Surgery inlaid treatment. Results Seven patients with residual shunt after congenital heart disease were placed a plug or coil closure of the residual shunt disappeared, 2 cases of ventricular septal defect with patent ductus arteriosus in children with a coil before the closure of patent ductus arteriosus , 7 days after interventional treatment of surgical treatment. A case of ventricular septal defect with pulmonary stenosis in children and a case of atrial septal defect with tricuspid regurgitation in patients with echocardiography evaluation of immediate effect was satisfactory. One case of echocardiography to guide the huge atrial septal defect involved in the failure of closure, the immediate surgical success. Conclusion Echocardiography is sensitive and convenient. It can guide the congenital heart disease in real time and provide important information for the operation. Evaluate the curative effect immediately after the operation, which is safe and reliable and plays an important role in the mosaic therapy of congenital heart disease.
