目的探讨累积高血压暴露与臂踝动脉脉搏波传导速度(baPWV)的关系。方法在参加开滦集团2006-2007年度健康体检的职工中按抽样标准分层随机抽取观察对象,符合入选标准者5440人,于2012-2013年对该人群进行了第4次健康体检,并检测了该人群的baPWV,最终有3676名观察对象纳入统计分析。累积血压暴露值(cumBP)包括累积收缩压暴露值(cumSBP)和累积舒张压暴露值(cumDBP),采用逐步线性回归和多因素Logistic回归方法,分析cumSBP和cumDBP与baPWV的关系及累积高血压暴露对baPWV的影响。结果在观察对象3676人中,年龄(55.7±9.8)岁,其中男性2012人,占54.7%,女性1664人,占45.3%。随着累积高血压暴露时间的增加,baPWV和baPWV≥14m/s的检出率呈上升趋势(均P<0.05)。在逐步线性回归分析中,校正其他混杂因素后,结果发现,cumSBP和cumDBP与baPWV仍呈正线性相关,β值分别为0.20和0.14。在多因素Logistic回归分析中,校正其他混杂因素后,结果发现,累积高血压暴露2年组、4年组、6年组、8年组与暴露0年组比较,均是发生baPWV≥14m/s的危险因素,OR值(95% CI)分别为1.77(1.39~2.26)、2.05(1.53~2.75)、3.20(2.22~4.61)、3.05(1.96~4.74)。结论 cumSBP和cumDBP与baPWV存在正线性相关,累积高血压暴露是baPWV升高的危险因素。
Objective To investigate the relationship between cumulative hypertension and brachial-ankle pulse wave velocity (baPWV). Methods A total of 5440 workers who participated in the health examination of Kailuan Group from 2006 to 2007 were stratified randomly according to the sampling criteria. The fourth physical examination was conducted in 2012-2013 and the results were tested The baPWV of this population, the final 3676 subjects included in the statistical analysis. Cumulative blood pressure (cumBP), including cumulative systolic blood pressure (cumSBP) and cumulative diastolic blood pressure (cumDBP), using stepwise linear regression and multivariate Logistic regression analysis of cumSBP and cumDBP relationship with baPWV and cumulative hypertension exposure Impact on baPWV. Results Among 3676 subjects, the age was (55.7 ± 9.8) years old, of which, 2012 was male, accounting for 54.7% and female 1664, accounting for 45.3%. The detection rate of baPWV and baPWV≥14 m / s increased with the increase of exposure time of cumulative hypertension (all P <0.05). In the stepwise linear regression analysis, after correcting for other confounding factors, it was found that there was a positive linear correlation between cumSBP and cumDBP and baPWV with β values of 0.20 and 0.14, respectively. In the multivariate Logistic regression analysis, after adjusting for other confounding factors, it was found that the incidence of baPWV≥14m / s was higher in 2-year, 4-year, 6-year, 8-year and 8- s. The odds ratio (95% CI) was 1.77 (1.39-2.26), 2.05 (1.53-2.75), 3.20 (2.22-4.61) and 3.05 (1.96-4.74), respectively. Conclusion There is a positive linear correlation between cumSBP, cumDBP and baPWV. Cumulative hypertension is a risk factor for the increase of baPWV.
