目的 :比较计算机辅助细胞检测系统 (computer assistedcytologytest ,CCT)与传统光镜在宫颈涂片细胞学诊断中的差异性。方法 :采用双盲法分别用CCT检测和传统光镜方法对 936例宫颈涂片作细胞学诊断 ,并对 6 5例进行阴道镜下取活组织病理检查。结果 :CCT检测阳性率为 8 76 % ( 82 936 ) ;传统光镜诊断阳性率为 5 45 % ( 5 1 936 ) ,两种方法的阳性检出率差异有显著意义 (P <0 0 1) ;CCT诊断的敏感性为 96 % ,特异性为 92 3 % ,漏诊率为4% ,传统光镜诊断的敏感性为 87 8% ,特异性为 81 8% ,漏诊率为 12 2 %。结论 :CCT诊断与传统光镜诊断结果比较 ,CCT的阳性检出率明显高于传统光镜 ,大大提高了诊断的准确性 ,为宫颈癌早期诊断提供了新的途径
OBJECTIVE: To compare the differences of cytological diagnosis of cervical smears between computer assisted cytology (CCT) and traditional light microscopy. Methods: A total of 936 cervical smears were examined by CCT and traditional light microscopy in a double-blind method. Cytological examination was performed on 65 cases under colposcopy. Results: The positive rate of CCT was 8 76% (82 936). The positive rate of conventional light microscopy was 5 45% (5 1 936). There was significant difference between the two methods (P 0 01) . The diagnostic sensitivity of CCT was 96%, the specificity was 92.3%, the rate of misdiagnosis was 4%. The sensitivity and specificity of traditional light microscopy were 87.8%, 81.8% and 12.5% respectively. Conclusion: Compared with traditional light microscopy, the positive rate of CCT is higher than that of traditional light microscopy, which greatly improves the diagnostic accuracy and provides a new way for early diagnosis of cervical cancer
