目的探讨老年代谢综合征(MS)患者铁蛋白与胰岛素抵抗和胰岛β细胞功能的关系。方法 112例老年患者分为有MS(A组,54例)和非MS(B组,58例),比较两组的体重指数(BMI)、铁蛋白、血脂、空腹胰岛素、空腹血糖、胰岛素抵抗指数(HOMA-IR)及胰岛β细胞功能指数(HOMA-β)。分析MS患者铁蛋白与胰岛素抵抗和胰岛β细胞功能的关系。结果A组铁蛋白水平高于B组[(219.54±128.33)ng/ml vs.(163.40±125.11)ng/ml](P<0.05)。铁蛋白与空腹血糖、三酰甘油、BMI和HOMA-IR呈正相关(r=0.333、0.190、0.126和0.360,P<0.01),而与HOMA-β呈负相关(r=-0.306,P<0.01)。铁蛋白与胰岛素抵抗独立相关。结论老年MS患者体内铁超负荷;铁蛋白升高可引起胰岛素抵抗增加及胰岛β细胞功能下降。
Objective To investigate the relationship between ferritin and insulin resistance and pancreatic β-cell function in elderly patients with metabolic syndrome (MS). Methods One hundred and twelve elderly patients were divided into MS group (A group, 54 cases) and non-MS group (B group, 58 cases). The body mass index (BMI), ferritin, serum lipid, fasting insulin, fasting blood glucose, insulin resistance Index (HOMA-IR) and pancreatic β-cell function index (HOMA-β). To analyze the relationship between ferritin and insulin resistance and pancreatic β-cell function in MS patients. Results The level of ferritin in group A was higher than that in group B [(219.54 ± 128.33) ng / ml vs. (163.40 ± 125.11) ng / ml] (P <0.05). Ferritin was positively correlated with fasting plasma glucose, triglyceride, BMI and HOMA-IR (r = 0.333,0.190,0.126 and 0.360, P <0.01), but negatively correlated with HOMA- ). Ferritin is independently associated with insulin resistance. Conclusion In elderly MS patients with iron overload, elevated ferritin may lead to increased insulin resistance and decreased pancreatic β-cell function.
