目的:探讨糖尿病脑血管病变的发病机理及其与中医毒损脑络学说的相互关系,为开展中医药对糖尿病脑血管病的有效防治提供科学依据。方法:实验分正常对照组、糖尿病组、中药糖宁胶囊治疗组和西药糖适平治疗组。以链尿佐菌素诱导糖尿病大鼠并连续观察24周,同时给予中药和西药灌胃治疗,采用激光衍射法测定大鼠外周血中红细胞变形性,运用放免法测定大鼠脑内TNF-α和ET-1含量;以化学发光法测定脑组织超氧化物歧化酶(SOD)活性,以硫代巴比妥酸法测定丙二醛(MDA)水平。结果:与对照组相比,长期糖尿病患鼠脑中YNF-α、ET-1和MDA含量增高,SOD活性降低,同时外周血中ET-1含量增高,红细胞变形性低下,差异非常显著(p<0.01);糖宁胶囊可有效降低糖尿病大鼠脑内TNF-α、ET-1和MDA含量,提高SOD活性和红细胞变形能力,与模型组比有显著差异(p<0.05-p<0.01)。结论:糖尿病脑血管病变的形成与中医毒损脑络的发病学说密切相关,活血解毒法及组方糖宁胶囊可有效延缓糖尿病脑血管病变,其机理与其提高红细胞变形能力,抑制TNF-α、ET-1的过量释放,保护血管内皮,以及清除氧自由基,减轻脂质过氧化损伤有关。
Objective: To investigate the pathogenesis of diabetic cerebrovascular disease and its relationship with the theory of toxic brain damage in Chinese medicine so as to provide a scientific basis for effective prevention and treatment of diabetic cerebrovascular disease by Chinese medicine. Methods: The experiment was divided into normal control group, diabetic group, traditional Chinese medicine Tang Ning capsule treatment group and Xi Yao Tang Shi Ping treatment group. Streptozotocin-induced diabetic rats were continuously observed for 24 weeks. Simultaneously with traditional Chinese medicine and Western medicine, the red blood cell deformability was measured by laser diffraction. Rat brain TNF-α was measured by radioimmunoassay. And ET-1 content; Determination of superoxide dismutase (SOD) activity in the brain by chemiluminescence method, and malondialdehyde (MDA) level by thiobarbituric acid method. Results: Compared with the control group, the levels of YNF-α, ET-1 and MDA in the brain of chronically diabetic mice increased, and the activity of SOD decreased. At the same time, the content of ET-1 in peripheral blood was increased and the red blood cell deformability was low. The difference was significant (p <0.01); G-Ning capsule can effectively reduce the content of TNF-α, ET-1 and MDA in brain of diabetic rats, and increase SOD activity and erythrocyte deformability, which is significantly different from the model group (p<0.05-p<0.01) . Conclusion: The formation of diabetic cerebrovascular disease is closely related to the pathogenesis of traditional Chinese medicine poisoning brain network. Huoxue Jiedu method and Zhifangfang Tang Ning capsule can effectively delay diabetic cerebrovascular disease, and its mechanism is to increase red blood cell deformability and inhibit TNF-α, Excessive release of ET-1 protects the vascular endothelium and scavenges oxygen free radicals and reduces lipid peroxidation damage.
