目的探讨彩色多普勒超声对下肢动脉硬化闭塞症(ASO)胫前动脉窃血的诊断价值。方法选取伴有胫前动脉窃血的ASO患者30例(病例组),同期健康志愿者30例(对照组),超声检测双侧胫前动脉、内踝前动脉、外踝前动脉、内踝后动脉、外踝后动脉。记录各动脉内径、血流方向、峰值流速。结果 30例48条胫前动脉窃血患者中,同侧内踝前动脉、外踝前动脉均逆流共43条,内踝前动脉逆流3条,外踝前动脉逆流2条。病例组内踝前动脉、外踝前动脉、内踝后动脉、外踝后动脉管腔较对照组增宽,流速增高,差异有统计学意义(P<0.05,P<0.01)。结论彩色多普勒超声能够准确地判断胫前动脉窃血患者的内外踝动脉网血流动力学变化情况,为临床选择治疗方案提供参考依据。
Objective To investigate the value of color Doppler ultrasound in the diagnosis of lower extremity arteriosclerosis obliterans (ASO) anterior tibial artery steal. Methods Thirty ASO patients with thrombosis of the anterior tibial artery (case group), 30 healthy volunteers (control group), bilateral anterior tibial artery, anterior malleolus, anterior malleolus, posterior malleolus, Lateral malleolus artery. Record the diameter of each artery, blood flow direction, peak flow rate. Results In 48 cases of 48 patients with anterior tibial artery steal, the anterior artery of ipsilateral medial malleolus and anterior lateral articular artery all had a total of 43 countercurrent reflux, 3 anterior ankle anterior reflux and 2 anterior cruciate ankle. The ankle anterior artery, ankle anterior artery, posterior medial malleolus artery and posterior lateral malleolus artery were broadened and the velocity of flow increased in the case group compared with the control group (P <0.05, P <0.01). Conclusions Color Doppler ultrasound can accurately determine the hemodynamic changes of the ankle and ankle maneuver in patients with anterior tibial artery steal and provide a reference for the clinical choice of treatment.
