目的 观察体外合成的 HPV16 L2抗原肽冲击的自身树突细胞免疫治疗 ,对人乳头瘤病毒感染致顽固生殖器尖锐湿疣的体液免疫学 ,细胞免疫学及病理学作用及疗效 .方法 10 9例反复复发 3次以上 ,常规治疗无效的顽固尖锐湿疣患者 ,随机分为两组 ,按知情同意原则 ,分别予常规疗法如干扰素或 L2抗原肽冲击的树突细胞免疫治疗 .全体患者随访 6 mo,疗程前后分别取外周血检测患者体液及细胞免疫功能 ,同时疗程前后分别取疣体 ,行 PCR,HE及免疫组化染色观察 .结果 L2抗原肽冲击的自身树突细胞免疫治疗后 ,患者体液及细胞免疫功能较普通治疗方法有显著改善 ,病灶处组织中大量免疫细胞浸润 ,空泡细胞减少 ,病毒感染清除或减轻 .治疗组复发率降至 10 % .结论 L2抗原肽冲击的树突细胞能刺激特异性体液免疫 ,改善反复复发、顽固性人乳头瘤病毒感染的生殖器尖锐湿疣患者的细胞免疫功能及病变局部免疫状态 ,对清除疣体部位病毒 ,防止复发有显著作用 .
