目的:探讨p53和c-erbB-2的表达阳性率和强度与胃癌浸润深度的关系。方法:收集139例胃癌手术切除标本,包括黏膜内癌23例、浸润至黏膜下层31例、浸润至肌层47例、浸润至浆膜或浆膜外38例,切片采用免疫组化S-P法染色,观察p53和c-erbB-2的表达阳性率和强度。结果:不同浸润深度的胃癌中p53的阳性率比较,黏膜内癌组和浸润至黏膜下层组与浸润至浆膜层或浆膜外组存在显著差异。而各组c-erbB-2的表达阳性率比较也均存在显著差异(P<0.01)。p53的表达强度在不同的浸润深度无显著差别(P>0.05),而c-erbB-2差别明显,在胃癌浸润至浆膜或浆膜外时,其表达强度最高(P<0.01)。c-erbB-2表达阳性率与p53存在相关性(P<0.05),而表达强度则无明显相关性(P>0.05)。p53和c-erbB-2的不同表达强度中,p53Ⅲ、c-erbB-2Ⅰ、c-erbB-2Ⅱ和c-erbB-2Ⅲ对胃癌浸润深度均有影响(P分别为0.001、0.010、0.000、0.000)。结论:p53和c-erbB-2在胃癌中的表达不是同步的,p53突变有利于c-erbB-2的表达,但c-erbB-2的表达强度与p53无关。抑癌基因p53突变可能与胃癌的起源有关;胃癌的浸润主要与癌基因c-erbB-2的激活有关。
Objective: To investigate the relationship between the positive expression rate of p53 and c-erbB-2 and the invasion depth of gastric cancer. METHODS: Thirteen gastric cancer specimens were collected, including 23 cases of intramucosal carcinoma, 31 cases of infiltration into the submucosa, 47 cases of infiltration into the muscular layer, 38 cases of infiltration into the serosa or serosa, and the sections were stained with immunohistochemical SP method The positive rate and intensity of p53 and c-erbB-2 expression were observed. Results: The positive rates of p53 in gastric cancer with different depth of invasion were significantly different between the intramucosal carcinoma and the submucosal invasion and the serosa or serosa. The positive rate of c-erbB-2 in each group also showed significant difference (P <0.01). There was no significant difference in the expression of p53 between different depths of invasion (P> 0.05). However, the expression of c-erbB-2 was significantly different between the two groups (P <0.01). The positive rate of c-erbB-2 was correlated with p53 (P <0.05), while there was no significant correlation between the expression of c-erbB-2 and p53 (P> 0.05). The expressions of p53Ⅲ, c-erbB-2Ⅰ, c-erbB-2Ⅱ and c-erbB-2Ⅲwere all different in the depth of invasion of gastric cancer (P = 0.001,0.010,0.000,0.000 ). Conclusions: The expression of p53 and c-erbB-2 in gastric cancer is not synchronous. The mutation of p53 is beneficial to the expression of c-erbB-2, but the expression of c-erbB-2 is not related to p53. The mutation of tumor suppressor gene p53 may be related to the origin of gastric cancer. The infiltration of gastric cancer is mainly related to the activation of oncogene c-erbB-2.
