目的:探讨自拟减毒汤加减联合康复新对鼻咽癌患者放疗不良反应的减毒效果观察。方法:选取2013年7月—2015年7月接受放射治疗的鼻咽癌患者96例(完成86例,脱落10例)作为研究对象,采用随机数字表法分为观察组(48例,完成44例,脱落4例)和对照组(48例,完成42例,脱落6例),对照组给予康复新治疗,观察组在对照组基础上给予自拟减毒汤加减治疗,以放疗结束作为干预终点,记录两组放射不良反应发生率、发生时间、持续时间,采用卡氏评分(KPS)评估功能状态,计算体质量指数(BMI)。结果:观察组口腔黏膜损伤、放射性食管炎、放射性皮肤损伤、口干的Ⅲ~Ⅳ级发生率分别为20.45%、9.09%、9.09%、6.82%,明显低于对照组的42.86%、28.57%、35.71%、30.95%;观察组口腔黏膜损伤、放射性食管炎、放射性皮肤损伤、口干的出现时间长于对照组,口腔黏膜损伤、放射性食管炎、放射性皮肤损伤持续时间短于对照组(P<0.05);观察组治疗后KPS评分、BMI高于对照组(P<0.05)。结论:自拟减毒汤加减联合康复新可减轻鼻咽癌患者放疗不良反应严重程度,延长不良反应出现时间,减少对生活质量、体质量的影响。
Objective: To observe the attenuated effect of self-made attenuated decoction combined with rehabilitation rehabilitation on patients with nasopharyngeal carcinoma radiotherapy. Methods: Ninety-six patients (86 completed, 10 off) with nasopharyngeal carcinoma treated with radiotherapy from July 2013 to July 2015 were enrolled in this study. The patients were randomly divided into observation group (48 cases, complete 44 (4 cases, 4 cases off) and control group (48 cases, complete 42 cases, 6 cases off). The control group was given Kang Fuxin treatment. The observation group was given the self-made attenuated yangtang decoction on the basis of the control group. The incidence, incidence and duration of adverse reactions in both groups were recorded. The functional status was assessed by Kappa score (KPS), and body mass index (BMI) was calculated. Results: The incidence of grade Ⅲ ~ Ⅳ of oral mucosal injury, radiation esophagitis, radiation skin injury and dry mouth were 20.45%, 9.09%, 9.09% and 6.82% in the observation group, which were significantly lower than those in the control group 42.86% and 28.57% , 35.71%, 30.95%. The appearance time of oral mucosal injury, radiation esophagitis, radiation skin injury and dry mouth of the observation group was longer than that of the control group. The duration of oral mucosal injury, radiation esophagitis and radiation skin injury was shorter than that of the control group (P < 0.05). KPS score and BMI in observation group were higher than those in control group (P <0.05). Conclusion: The self-made attenuated decoction combined with rehabilitation rehabilitation can reduce the severity of adverse reactions of radiotherapy in patients with nasopharyngeal carcinoma, prolong the emergence of adverse reactions, reduce the impact on quality of life and body weight.
