目的:评估乳腺导管内视镜对乳腺导管内新生物的诊断价值。方法:对乳头溢液病人进行乳管内视镜检查,对其中乳管内发现新生物者行手术治疗,对它们在乳管内视镜下的特点予以分析,总结乳管镜诊断与病理组织学诊断间相互关系。结果:624例乳头溢液患者行乳管镜检查,其中156例导管内发现新生物:单发性乳头状瘤136例;乳头状瘤病10例;导管内癌10例。其中28.2%的病灶位于总导管;45.6%位于导管的I级分支,17.9%位于II级分支,位于III级分支和IV级分支病灶仅占6.9%和1.4%。病灶距乳头开口的平均距离为2.5cm。162例均行手术治疗。术后病理诊断证实:130例为乳头状瘤,6例为导管炎,10例为乳头状瘤病,10例为导管内癌。乳管镜诊断符合率:乳头状瘤91.9%,乳头状瘤病100%,导管内癌100%。结论:乳管内视镜是术前诊断以乳头溢液为表现的乳腺管内新生物有效的诊断手段。
OBJECTIVE: To evaluate the diagnostic value of ductal endoscopy in the diagnosis of neoplasms in the mammary duct. Methods: The patients with nipple discharge were examined by endoscopy, and the neoplasms found in the ducts were surgically treated. The characteristics of them under the endoscopy were analyzed. The relationship between ductal diagnosis and histopathological diagnosis Interrelationship. Results: 624 cases of nipple discharge patients underwent ductoscopy, of which 156 cases of new biological found in the catheter: solitary papilloma in 136 cases; 10 cases of papilloma; ductal carcinoma in 10 cases. Among them, 28.2% of the lesions were located in the total duct; 45.6% were in the grade I branch of the duct, 17.9% were in the grade II branch, and those in the grade III and IV branches accounted for only 6.9% and 1.4% of the lesions. The average distance from the lesion to the nipple opening was 2.5 cm. 162 cases were treated surgically. Postoperative pathological diagnosis confirmed: 130 cases of papilloma, 6 cases of catheteritis, 10 cases of papillomatosis, 10 cases of intraductal carcinoma. Diagnosis of ductal mirror coincidence rate: 91.9% of papilloma, papilloma 100%, ductal carcinoma 100%. Conclusion: The ductal endoscopy is a new biologically effective diagnostic method for preoperative diagnosis of breast duct with nipple discharge.
