胰岛素注射液致过敏反应1例

胰岛素为降血糖药,主要药效为降血糖,同时影响蛋白质和脂肪代谢。本病例使用的制剂为胰岛素(猪)的灭菌水溶液,辅料为甘油,苯酚(每100 ml中含0.25 g)。现将我院1例胰岛素注射液致过敏反应报道如下。患者,男,56岁,查体发现左侧肾盂、左侧输尿管上段占位性病变3个月。B超示:左肾及左侧上段输尿管低回声,左肾重度积水。CT平扫示:左侧肾盂、左侧输尿管上段占位性病变。门诊以“左输尿管占位”收入院。入院后患者血糖、血压偏高,随机血糖检测值为13.2 mmol·L~(-1),监测血糖、血压。并行左侧肾盂输尿管癌根治术,术后转入ICU。患者既往血糖高,自服“二甲双胍”控制不理想。入院体检:T36.4℃、BP 150/90 mmHg,心肺腹未见明显异常,双肾区无叩击痛,双肾未扪及。沿双侧输尿管走行区双侧无压痛,未扪及肿块。耻骨上膀胱区不充盈,无压痛。肛门指诊:前列腺稍大,中央沟存在,质韧,无触痛,未触及明显硬结。辅助检查:肾图左肾未显影,左肾几无灌注、无功能;右肾血 Insulin is a hypoglycemic drug, the main effect is hypoglycemic, while affecting protein and fat metabolism. The preparation used in this case is a sterile aqueous solution of insulin (pig) with glycerin and phenol (0.25 g per 100 ml). Now in our hospital 1 case of insulin injection caused by allergic reactions are reported below. Patients, male, 56 years old, physical examination found the left renal pelvis, left upper ureteral lesions accounted for 3 months. B ultrasound showed: the left renal and left upper ureter hypoechoic, left kidney severe hydronephrosis. CT scan showed: the left renal pelvis, upper left ureteral mass lesions. Out-patient to “left ureter placeholder ” income hospital. Blood glucose and high blood pressure were higher in patients after admission. The measured value of random blood glucose was 13.2 mmol·L -1, and blood glucose and blood pressure were monitored. Parallel left ureteropelvic cancer radical surgery, transferred to the ICU. Patients with high blood sugar, self-service “metformin ” control is not ideal. Admission examination: T36.4 ℃, BP 150/90 mmHg, no obvious cardiopulmonary abdominal abnormalities, no perineal percussion pain, no palpable renal. Bilateral bilateral ureteral walking along the bilateral no tenderness, no palpable mass. Suprapubic bladder area is not full, no tenderness. Anal fingering: prostate is slightly larger, the central groove exists, quality toughness, no tenderness, no obvious induration.
