二点半综合征是一种T波的正常变异。是指休息状态时,心电图QRS波额面平均电轴+90°,T波电轴-30°。若以QRS波电轴代表时钟的长针,T波电轴代表时钟的短针,正好是二点半,故称为“二点半综合征”(Half-Past-Two syndrome)。 T波的正常变异可见于精神的焦虑或恐惧,迷走神经张力的增加,体位的改变,饭后或通气过度所引起。二点半综合征为T波的正常变异之一。在正常的情况下,QRS波和T波的向量不论在额面或水平面二者的方向基本上是一致的。QRS-T夹角是比较小的。以额面的六轴系统来计算QRS-T夹角一般是小于45°,极少数超过60°。二点半综合征者QRS波电轴指向+90°,T波轴指向-30°。QRS-T夹角可达到110°。这种T波变化,甚为少见,可出现在正常人,尤其是体型瘦长者。运动后QRS轴仍指向+90°,而T波轴可顺时钟转向到+30°,但2分钟后T波轴又逐渐转列-30°附近。由于此种T渡变化很易误诊为器质性心脏病,故临床应予注意。
