目的 :研究血管紧张素Ⅱ 1型受体 (AT1R)A1166C多态及血管紧张素Ⅰ转化酶 (ACE)基因I/D多态与中国汉族 2型糖尿病肾病 (DN)的关系。方法 :运用PCR和PCR/DdeI酶切技术 ,检测 111例DN患者与 138例无DN患者AT1R基因及ACE基因多态基因型。结果 :DN组和无DN组相比 ,AT1R基因A1166C多态基因型频率无差异 (P >0 0 5 ) ;2组间AT1R基因A1166C多态等位基因频率也无差异 (P >0 0 5 ) ;DN组ACE基因DD基因型和D型等位基因频率均升高 (P <0 0 5 ) ;ACE基因DD基因型在AT1R基因AA、AC和CC 3种基因型患者中的分布有差异 (χ2 =7 82 ,P <0 0 5 )。结论 :ACE基因I/D多态性与 2型糖尿病并发DN有关 ,携带DD基因型和D型等位基因的 2型糖尿病患者是DN的易感人群。AT1R基因A1166C多态性不参与 2型糖尿病DN的发病。ACE基因I/D多态和AT1R基因A1166C多态在DN的发生、发展中均为微效基因 ,AA、AC、CC基因型患者携带D型等位基因易发生DN ,ACE、A1166C多态和ACE基因I/D多态存在协同效应
Objective: To investigate the relationship between the A1166C polymorphism of angiotensin Ⅱ type 1 receptor (AT1R) and the polymorphism of angiotensin Ⅰ converting enzyme (ACE) gene and type 2 diabetic nephropathy (DN) in Chinese Han population. Methods: The genotypes of AT1R gene and ACE gene in 111 patients with DN and 138 patients without DN were detected by PCR and PCR / DdeI digestion. Results: There was no difference in the frequency of A1166C polymorphism in AT1R gene between DN group and DN group (P> 0.05). There was no difference in the frequencies of A1166C polymorphism allele between two groups (P> 0.05) ). The frequencies of DD genotype and D genotype of ACE gene in DN group were significantly increased (P <0.05). There were differences in the distribution of ACE genotype DD genotype among AT1R gene AA, AC and CC genotypes (χ2 = 7 82, P <0 05). CONCLUSION: ACE gene I / D polymorphism is associated with type 2 diabetes mellitus complicated with DN. Patients with type 2 diabetes who carry DD genotype and D-type allele are susceptible to DN. AT1R gene A1166C polymorphism is not involved in the pathogenesis of type 2 diabetes DN. ACE gene I / D polymorphism and A1166C polymorphism of AT1R gene are all micro-gene in the occurrence and development of DN. DN, ACE, A1166C polymorphism are more likely to occur in patients with AA, AC, CC genotypes carrying D-type allele ACE gene I / D polymorphism exists synergistic effect
