【目的】探讨血清肿瘤标记物与C‐反应蛋白(CRP)等对儿童神经母细胞瘤(NB)疗效评价及预后评估的价值。【方法】对80例确诊为NB患儿的临床资料进行回顾性分析,对比治疗前后,肿瘤进展或复发前后血清神经元特异性烯醇化酶(NSE)、血清铁蛋白(SF)、乳酸脱氢酶(LDH)及CRP水平的变化;比较初诊时早期与晚期NB ,以及不同原发部位的 NB患儿血清 NSE、SF、LDH 及CRP水平的差异;分析血清 NSE与SF、LDH、CRP水平的相关性。【结果】治疗有效后血清NSE、SF、LDH及CRP水平显著降低( P <0.001);病情进展或复发后血清NSE、SF、LDH及CRP水平明显上升( P <0.05);晚期NB血清NSE、SF、CRP水平明显高于早期( P <0.05),晚期NB血清LDH高于早期,但无统计学意义( P >0.05);腹部NB血清NSE、SF、LDH、CRP水平明显高于其他部位 NB( P <0.05)。初诊时血清 NSE>100μg/L 提示预后不良( P <0.05)。血清NSE与SF( r=0.765,P<0.05)、LDH( r=0.472,P<0.001)、CRP( r=0.38,P<0.001)均呈正相关。【结论】血清NSE对NB的病情监测、疗效评价及预后评估有一定的临床价值。“,”[Objective] To explore the clinical values of serum neuron specific enolase (NSE) in the man‐agement of neuroblastoma (NB) .[Methods]The clinical data were retrospectively analyzed for 80 NB children to compare the changes of NSE ,serum ferritin (SF) ,lactate dehydrogenase (LDH) and C‐reactive protein (CRP) before and after treatment ,tumor progression or recurrence .And the serum levels of NSE ,SF ,LDH and CRP were analyzed for early and late initial visits and different primary tumor sites .And correlation stud‐ies were performed for the above parameters .[Results]The serum levels of NSE ,SF ,LDH and CRP signifi‐cantly declined after treatment ( P <0 .001) .After progression or recurrence ,the serum levels of NSE ,SF and LDH significantly increased ( P <0 .05) .The serum levels of NSE ,SF and CRP for advanced tumors were significantly higher than those for early tumors ( P 0 .05) .The serum levels of NSE ,SF ,LDH and CRP were higher in patients with intra‐ab‐dominal primary sites than those with other sites ( P 100 μg/L was a predicator of poor prognosis ( P <0 .05) .And the serum level of NSE was positively correlated with those of SF ( r =0 .765 ,P<0 .05) ,LDH ( r =0 .472 ,P<0 .001) and CRP ( r =0 .38 ,P<0 .001) .[Con‐clusion] Serum NSE has some clinical values in monitoring ,efficacy assessment and outcome prediction of NB .
