男患,82岁,不明原因咳嗽、气喘,着凉时加重3～4年。近三月来症状加重伴咳痰,有少量血痰。病前无特殊服药史。查体:T37.4℃,p120次,R30次,Bp13.3/6.67kPa。重度贫血貌,浅表淋巴结不肿大,巩膜轻度黄染。胸骨无压痛,肋间隙饱满,心界缩小,心律齐,两肺(-),肝脾未触及。血常规:Hb73g/L。RBC2.2×10~(12)/L,WBC1.2×10~(?)/L.N 0.86,L 0.14,网织红细胞0.01,骨髓象:呈增生性贫血。黄疸指数9u,血总胆红质0.4mg,热溶血试验、Hamm试验阴性,粪潜血(+),静脉血标本在37℃盐水中不凝集,在4℃冰箱过夜呈现凝集,血清凝集素测定:在4℃1小时为1:256,在4℃14小时为1:2048。同
