目的:研究人脑动静脉畸形(CAVM)血管平滑肌细胞(VSMC)的表型转变。方法:以骨桥蛋白作为合成型VSMC表型的标志,对26例CAVM病理标本及对照组10名正常脑组织的血管进行免疫组化分析。结果:26例CAVM畸形血管中22例有骨桥蛋白不同程度的表达,主要见于CAVM的静脉部分,尤多见于管壁不规则增厚的静脉。在类似粥样硬化样病变的动脉处也有表达。正常脑组织的血管中未见骨桥蛋白表达。按有无出血史及畸形血管团大小分组进行比较,未发现骨桥蛋白的表达在组间差异没有统计学意义(P>0.05)。结论:人CAVM血管壁中部分VSMC转变为合成表型,这是CAVM的血管适应血流动力学状态发生的进行性血管重塑的结果。
Objective: To investigate the phenotypic changes of vascular smooth muscle cells (VSMC) in human cerebral arteriovenous malformations (CAVM). Methods: Osteopontin was used as a marker of synthetic VSMC phenotype. Immunohistochemical analysis was performed on the blood vessels of 26 CAVM pathological specimens and 10 normal brain tissues from the control group. RESULTS: Twenty-two cases of osteopontin in 26 cases of CAVM had the expression of osteopontin with different degrees, mainly found in the vein of CAVM, especially in the vein with irregular thickening of the wall. Atherosclerosis-like lesions also have expression. No osteopontin expression was observed in normal brain tissue. There was no history of bleeding and malformation vascular group size group comparison, did not find the expression of osteopontin in the group difference was not statistically significant (P> 0.05). CONCLUSIONS: Part of the vascular wall of human CAVM transforms to a synthetic phenotype, which is the result of progressive vascular remodeling of the vascular adaptive hemodynamics of the CAVM.
