利用录象脑电图加强监测, ̄(99m)Tc-HMPAO-SPECT, ̄(123)J-Iomazenil-SPECT、MRI对40例局限性癫痛患者进行研究。分析发作期及发作间歇期Video/EEG的结果:38例(82%)表现局限性癫痛样活动, ̄(99m)TC-HMPAO-SPECT局部脑血流异常21例(52%), ̄(123)J-Iomazenil-SPECT局部脑苯二氮革受体密度减低25例(63%),MRI结构异常18例(45%)。Iomazenil对癫痈灶定位比HMPAO敏感,有较高诊断价值。这些神经影像技术可显示病灶内结构和功能改变。对癫痈发作提供了神经病理学基础,是对癫痛定位非创伤性的有效检查方法。
Surveillance was performed using video-electroencephalography (EEG) in 40 patients with localized epilepsy under ~ (99m) Tc-HMPAO-SPECT and ~ (123) J-Iomazenil-SPECT. Video / EEG results from the onset and interictal episodes were analyzed: 38 (82%) exhibited localized epilepsy-like activity, ~ (99m) TC-HMPAO-SPECT had local cerebral blood flow abnormalities in 21 123) J-Iomazenil-SPECT local brain benzodiazepine receptor density decreased in 25 cases (63%), MRI structure abnormalities in 18 cases (45%). Iomazenil is more sensitive to epilepsy than HMPAO and has higher diagnostic value. These neuroimaging techniques can show structural and functional changes in the lesion. It provides a neuropathological basis for the onset of epilepsy and is a non-invasive and effective method of detecting epilepsy.
