目的:研究超声测量宫颈长度预测双胎自然早产的价值。方法:对52例孕28~34周双胎孕妇进行超声宫颈长度测量,并追踪这些孕妇的妊娠结局,统计分析使用该方法预测自然早产的敏感性、特异性、阳性预测值和阴性预测值等指标。结果:①52例28~34孕周的双胎孕妇中,宫颈长度<30 mm(设为阳性)10例,≥30 mm(设为阴性)42例。10例阳性者7天和孕37周前分娩率分别为40.0%和100.0%,差异有统计学意义。42例阴性孕妇7天和孕37周前均未分娩。②超声测量宫颈长度预测7天内双胎自然早产的敏感性、特异性、阳性预测值和阴性预测值分别为100.0%、87.5%、40.0%、100.0%;而在孕37周前上述指标均为100.0%,特异性和阳性预测值差异均有统计学意义。结论:超声测量宫颈长度是预测双胎早产极有价值的科学方法。若在28~34孕周期间双胎孕妇宫颈长度呈阴性,则一般不会发生早产;若呈阳性,则发生早产的可能性很高。
Objective: To study the value of uterine cervical length in predicting natural premature delivery of twins. Methods: 52 cases of pregnant women with gestational age from 28 to 34 weeks were measured with ultrasound cervical length and the pregnancy outcomes of these pregnant women were tracked. Statistical analysis was used to predict the sensitivity, specificity, positive predictive value and negative predictive value of natural preterm labor index. Results: Among the 52 pregnant women with gestational age from 28 to 34 weeks, 10 cases had cervical length less than 30 mm (positive) and 42 cases ≥30 mm (negative). 10 cases of positive 7 days and 37 weeks before delivery delivery rate was 40.0% and 100.0%, the difference was statistically significant. 42 cases of negative pregnant women 7 days and 37 weeks before pregnancy did not give birth. ②Ultrasonic measurement of cervical lengthThe sensitivity, specificity, positive predictive value and negative predictive value of prenatal twins were 100.0%, 87.5%, 40.0% and 100.0% within 7 days respectively; while the above indexes were all 37 100.0%, the difference between specificity and positive predictive value was statistically significant. Conclusion: Ultrasound measurement of cervical length is a valuable scientific method to predict preterm birth of twins. If during the 28-34 gestational week twin pregnancies have a negative cervical length, there is generally no premature birth; if it is positive, the likelihood of premature delivery is high.
