目的运用Meta分析方法探讨血清肿瘤坏死因子-α(TNF-α)在尘肺发生发展中的意义。方法通过计算机检索,收集所有有关尘肺患者血清TNF-α表达水平的文献,对符合标准的文献进行Meta分析,应用RevMan4.2软件进行异质性检验和效应值检验,用加权均数差(WMD)表示。结果纳入相关文献10篇。Meta分析结果显示,尘肺患者血清TNF-α表达水平与对照组之间的差异没有统计学意义(WMD=-0.66,95%CI:-3.82~2.49),对尘肺进行分期后发现,Ⅰ、Ⅱ、Ⅲ期尘肺患者血清TNF-α表达水平与对照组之间的WMD分别为9.43、-4.47和-16.12,并且发现Ⅲ期尘肺患者血清TNF-α表达水平与对照组之间的差异有统计学意义(WMD=-16.12,95%CI:-28.27~-3.97)。不同尘肺类型患者血清TNF-α表达水平与对照组之间的差异无显著性(P>0.05)。结论尘肺患者血清TNF-α表达水平与尘肺的发展有关,尘肺早期表达升高,尘肺晚期表达降低。
Objective To investigate the significance of serum tumor necrosis factor-α (TNF-α) in the development of pneumoconiosis by Meta-analysis. Methods All the literature about serum TNF-α levels in patients with pneumoconiosis were collected by computer. Meta-analysis was performed on the standard literatures, and the heterogeneity and test of effect were tested by RevMan4.2 software. The weighted mean difference (WMD ) Said. Meta analysis showed that there was no significant difference between the levels of serum TNF-αand the control group in patients with pneumoconiosis (WMD = -0.66,95% CI: -3.82-2.49). Conclusions The level of TNF-α in pneumoconiosis patients is related to the development of pneumoconiosis. The expression of pneumoconiosis in early stage of pneumoconiosis is increased and the expression of pneumoconiosis in late stage of pneumoconiosis is decreased.
