目的观察泡沫敷料联合重组人表皮生长因子凝胶在Ⅱ期和Ⅲ期压疮护理中的应用效果。方法选取2014—2015年保定市第一中心医院收治的Ⅱ期、Ⅲ期压疮患者153例,按照抛硬币法将患者分为对照组71例和观察组82例。两组患者均给予压疮基础护理和健康教育。对照组患者敷康惠尔渗液吸收贴,观察组患者取适量重组人表皮生长因子凝胶涂于创面,再敷康惠尔渗液吸收贴。比较两组患者治疗1周、2周、3周后新生肉芽组织成熟度,并在治疗后3周比较两组患者创面愈合时间、临床疗效及治疗1周羟脯氨酸(HYP)含量、Ⅰ/Ⅲ型胶原比例。结果两组患者治疗1周后创面新生肉芽组织成熟度比较,差异无统计学意义(P>0.05);而治疗2周、3周后,观察组患者创面新生肉芽组织成熟度均高于对照组(P<0.05)。治疗3周后,观察组患者临床疗效优于对照组(P<0.05),创面愈合时间短于对照组短(P<0.05)。治疗1周后观察组患者创面HYP含量高于对照组,Ⅰ/Ⅲ型胶原比例低于对照组(P<0.05)。结论泡沫敷料联合重组人表皮生长因子凝胶在Ⅱ期和Ⅲ期压疮护理中的应用效果确切,可促进创面新生肉芽组织成熟,缩短愈合时间。
Objective To observe the effect of foam dressing combined with recombinant human epidermal growth factor gel in stage Ⅱ and stage Ⅲ pressure ulcer care. Methods A total of 153 patients with stage Ⅱ and stage Ⅲ pressure ulcer admitted to the First Central Hospital of Baoding from 2014 to 2015 were selected and divided into control group (n = 71) and observation group (n = 82) according to the coin tossing method. Two groups of patients were given pressure ulcer basic care and health education. The control group of patients with Corneal exudate absorption patch, the observation group patients with appropriate amount of recombinant human epidermal growth factor gel applied to the wound, and then apply Kang Hui Er exudate absorption paste. The maturity of new granulation tissue was compared between two groups after 1 week, 2 weeks and 3 weeks. The wound healing time, the clinical effect and the level of HYP in 1 week, Ⅰ / Ⅲ Collagen ratio. Results There was no significant difference in the maturity of granulation tissue between the two groups after 1 week of treatment (P> 0.05). After 2 weeks and 3 weeks of treatment, the maturity of granulation tissue in the wound of the observation group was higher than that of the control group (P <0.05). After 3 weeks of treatment, the clinical efficacy of the observation group was better than that of the control group (P <0.05), and the wound healing time was shorter than that of the control group (P <0.05). After 1 week of treatment, the level of HYP in wounds in observation group was higher than that in control group, and the proportion of type Ⅰ / Ⅲ collagen was lower than that in control group (P <0.05). Conclusion The combination of foam dressing and recombinant human epidermal growth factor gel is effective in the treatment of stage Ⅱ and stage Ⅲ pressure ulcer wounds. It can promote the maturation of wound granulation tissue and shorten the healing time.
