目的:观察泄浊排毒方对痛风患者关节的保护效应。方法:将佛山市中医院收治的60例患者随机分为治疗组和对照组,各30例,治疗组给予泄浊排毒方加西乐葆治疗,对照组给予秋水仙碱加西乐葆治疗。观察患者症状缓解程度以及尿酸(UA)、C反应蛋白(CRP)、血沉(ESR)、白细胞介素-6(IL-6)的变化,采用超声仪对患者受累第一跖趾关节的滑膜厚度和血流信号RI进行观察。结果:两组均可改善关节疼痛、肿胀,缓解肿胀方面治疗组优于对照组,差异具有统计学意义(P<0.05)。两组治疗后(UA)、CRP、ESR均降低(P<0.05),且治疗组治疗后降低程度优于对照组(P<0.05)。治疗组的IL-6在治疗后比治疗前显著降低(P<0.05)。治疗后两组患者滑膜厚度均有显著改善,与治疗前比较,差异具有统计学意义(P<0.05),且治疗后的结果治疗组优于对照组,差异具有统计学意义(P<0.05)。两组治疗后血流信号RI均有显著改善(P<0.05)。结论:泄浊排毒方对痛风的关节保护有确切疗效,且在评价药物疗效时加入超声检查,将较以往提供更充实的研究依据,更形象且直观的反映泄浊排毒方治疗痛风的作用。
Objective: To observe the protective effect of Xiezhuo Paidu on the joint of gout patients. Methods: Sixty patients admitted to Foshan Hospital of Traditional Chinese Medicine were randomly divided into treatment group and control group, with 30 cases in each group. The treatment group was given Xiezhuo detoxification plus celecoxib and the control group was treated with colchicine plus celecoxib. The degree of remission and the changes of uric acid (UA), C-reactive protein (CRP), erythrocyte sedimentation rate (ESR) and interleukin-6 (IL-6) were observed. The synovial membrane of the first metatarsophalangeal joint Thickness and blood flow signal RI were observed. Results: The two groups can improve joint pain, swelling, relieve swelling in the treatment group than the control group, the difference was statistically significant (P <0.05). The UA, CRP and ESR of both groups were decreased (P <0.05), and the reduction of treatment group was better than that of the control group (P <0.05). IL-6 in the treatment group was significantly lower than that before treatment (P <0.05). After treatment, the synovial thickness of both groups improved significantly, with statistical significance (P <0.05), and the difference between the two groups after treatment was better than that of the control group (P <0.05) ). After treatment, the RI of blood flow was significantly improved (P <0.05). Conclusion: Xiezhuo Paidu decoction has the exact effect on the protection of gout, and adding ultrasound in evaluating the curative effect of the drug will provide more substantiated research evidence than ever before and more vividly and directly reflect the effect of Xiezhu Paidu decoction in treating gout.
