Human immunodeficiency virus type 1 (HIV-1) also actively replicates in patients with no clinical signs of infection. However, in the lymph nodes, which are the main source of extracellular virosomes, infected cells containing HIV-1 transcripts and intracellular viral proteins are rare. This phenomenon suggests that not only a very small number of cells involved in viral replication in vivo, but also the active site of HIV-1 replication was not found. In order to identify possible other viral replication sites, the authors studied 13 surgically resected tonsil specimens. There was no evidence of typical tonsil infection or tumor in the primary pathology analysis, but there was a marked enlargement of lymphoid follicles and multinucleated giant cells that would be recruited as a chronic infection. The patient’s serum has HIV-1 antibody, but only 2 people have AIDS symptoms.
