目的探讨血清淀粉样蛋白A(SAA)、C反应蛋白(CRP)与糖尿病合并冠心病的关系,为临床预防和治疗提供依据。方法 90例研究对象设为正常对照组(NC)、糖尿病合并冠心病组(DM+CHD)、糖尿病组(DM)及冠心病组(CHD)。测定受试者血清的SAA和CRP,然后与测定的白细胞计数(WBC)、空腹血糖(FPG)、空腹C肽(FCP)、糖化血红蛋白(HbA1C)、三酰甘油(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、收缩压(SBP)、舒张压(DBP)和体质指数(BMI)进行统计学分析。结果 DM+CHD组的SAA、CRP水平均显著高于DM组、CHD组、NC组,差异有统计学意义(P<0.01);SAA水平与SBP、DBP、FPG、HbA1C、TG、CRP、WBC呈正相关;CRP水平与BMI、SBP、DBP、FPG、HbA1C、TG、SAA、WBC呈正相关;多因素Logistic回归分析显示,糖尿病合并大血管病变与CRP、SAA、WBC、BMI有关。结论亚临床炎症可能参与糖尿病合并冠心病的发生和发展;SAA、CRP均与SBP、DBP、FPG、HbA1C、WBC呈正相关;SAA、WBC、CRP和BMI是糖尿病合并冠心病发生的危险因素。
Objective To investigate the relationship between serum amyloid A (SAA), C-reactive protein (CRP) and diabetes mellitus complicated with coronary heart disease (CHD) and to provide evidence for clinical prevention and treatment. Methods Ninety subjects were enrolled as control group (NC), diabetic patients with coronary heart disease (DM + CHD), diabetes mellitus (DM) and coronary heart disease (CHD) group. The levels of SAA and CRP in the serum of the subjects were determined and compared with the measured values of WBC, FPG, FCP, TG, TG, TC ), HDL-C, LDL-C, SBP, DBP and BMI were analyzed statistically. Results The levels of SAA and CRP in DM + CHD group were significantly higher than those in DM group, CHD group and NC group (P <0.01). The levels of SAA and SBP, DBP, FPG, HbA1C, TG, There was a positive correlation between CRP level and BMI, SBP, DBP, FPG, HbA1C, TG, SAA and WBC. Multivariate Logistic regression analysis showed that diabetic macroangiopathy was associated with CRP, SAA, WBC and BMI. Conclusions Sub-clinical inflammation may be involved in the occurrence and development of diabetes mellitus with coronary heart disease. SAA and CRP are positively correlated with SBP, DBP, FPG, HbA1C and WBC. SAA, WBC, CRP and BMI are risk factors for diabetes complicated with coronary heart disease.
