目的:采用放射性核素99mTc标记抗癌胚抗原(CEA)抗体C50进行放射免疫显像(RI)以探讨其临床应用价值。方法:①对152例临床怀疑为各种肿瘤的患者做了RI检查,包括卵巢肿瘤115例,肠道肿瘤26例和肺肿瘤11例。②用2巯基乙醇还原法标记99mTcC50,其标记率达90%以上,单抗用量1~15mg。③静注地塞米松防过敏,99mTcC50静脉注射后4~6h采集平面或断层图像。④以病灶处有放射性浓聚或T/NT比值大于12为阳性判断标准。⑤全部病例均做了病理检查,其中卵巢肿瘤组同时进行了B超诊断,肠道肿瘤组做了免疫组化实验。结果:99mTcC50在恶性病变灶中有较多的滞留,诊断敏感性882%,特异性832%,准确性为849%,转移灶的检出率达844%,其诊断效能明显优于B超。而RI的质量与血清CEA水平无关,与肿瘤的大小、肿瘤细胞CEA反应及肿瘤细胞分泌的CEA在组织中的分布有关。结论:认为99mTcC50RI对肿瘤的定性诊断和转移灶的检出有价值,是一种安全、简便和实用的影像诊断新方法
Objective: To investigate the clinical value of radioimmunoimaging (RI) using radionuclide 99mTc labeled anti-CEA antibody C50. Methods: (1) 152 cases of clinically suspected patients with various tumors RI examination, including 115 cases of ovarian tumors, intestinal tumors in 26 cases and lung tumors in 11 cases. ② 2-mercaptoethanol reduction labeled 99mTc C50, the labeling rate of 90% or more, the amount of monoclonal antibody 1 ~ 1 5mg. ③ intravenous dexamethasone anti-allergic, 99mTc C50 4 ~ 6h after intravenous injection plane or tomographic images. ④ to focus on radioactive accumulation or T / NT ratio greater than 1 2 as a positive criterion. ⑤ All cases were pathologically examined, including ovarian cancer group B ultrasound diagnosis, intestinal tumor group immunohistochemical experiments. Results: 99mTc C50 in malignant lesions have more retention, the diagnostic sensitivity of 88 2%, specificity 83 2%, the accuracy of 84 9%, the detection rate of metastasis was 84 4% , Its diagnostic efficiency is obviously superior to B-ultrasound. The quality of RI has nothing to do with the serum CEA level, and the size of the tumor, the CEA response of tumor cells and the distribution of CEA secreted by tumor cells in the tissue. Conclusion: 99mTc C50RI on the qualitative diagnosis of tumors and the detection of metastases valuable, is a safe, easy and practical new diagnostic imaging method
