目的探讨新疆地区维吾尔族(维族)和汉族帕金森病(Parkinson’s disease,PD)患者睡眠障碍差异性及PD患者睡眠状态的影响因素。方法 400例PD患者(汉族210例,维族190例)为PD组,400例成人体检健康者(汉族202例,维族198例)为对照组。2组完成基础情况调查表、临床症状评定量表及Hoehn&Yahn评分、匹兹堡睡眠质量指数(Pittsburgh Sleep Quality Index,PSQI)评分、广泛性焦虑量表-7项(Generalized Anxiety Disease-7,GAD-7)和健康问卷抑郁量表-9项(Patient Health Questionnaire-9,PHQ-9)评分,并检测血尿酸、血同型半胱氨酸等水平,比较维族及汉族PD患者睡眠状态差异性;采用相关分析和多元回归分析影响PD患者睡眠的可能因素。结果 PD组睡眠障碍发生率(27.2%)高于对照组(6.2%)(P<0.05),维族PD组睡眠障碍发生率(35.3%)与汉族PD组(20.0%)比较差异无统计学意义(P>0.05);Pearson相关线性分析示,PSQI总分与年龄(rs=0.719,P=0.033)、体质量指数(rs=0.307,P=0.023)、病程(rs=0.753,P=0.013)、GAD-7(rs=0.0.456,P=0.013)、PHQ-9(rs=0.621,P=0.040)呈正相关,经逐步回归,年龄、病程、PHQ-9进入方程。结论新疆地区PD患者睡眠障碍发生率高于正常人群,但维、汉族之间无明显民族差异性;PD患者年龄越大、病程越长、抑郁程度越重,其睡眠状态越差。
Objective To investigate the differences of sleep disorders between Uighur (Uighur) and Han patients with Parkinson’s disease (PD) in Xinjiang and the influencing factors of sleep status in PD patients. Methods One hundred and forty PD patients (210 Han and 190 Uygur) were selected as PD group and 400 healthy adults (202 Han and 198 Uygur) as controls. Group 2 completed the basic situation questionnaire, clinical symptom rating scale and Hoehn & Yahn score, Pittsburgh Sleep Quality Index (PSQI) score, Generalized Anxiety Disease-7 (GAD-7) And Patient Health Questionnaire-9 (PHQ-9), and to detect the levels of serum uric acid, blood homocysteine and so on, to compare the differences of sleep state among PD patients with Uygur and Han ethnicity. The correlation analysis And multiple regression analysis of possible factors affecting the sleep of PD patients. Results The incidence of sleep disorders in PD group was significantly higher than that in control group (27.2% vs 6.2%, P <0.05). The incidence of sleep disorders in Uygur PD group (35.3%) was not significantly different from that in Han PD group (20.0%) (P> 0.05). Pearson correlation analysis showed that the total score of PSQI was significantly correlated with age (rs = 0.719, P = 0.033), body mass index (rs = 0.307, , GAD-7 (rs = 0.0.456, P = 0.013) and PHQ-9 (rs = 0.621, P = 0.040) .Through stepwise regression, age, disease course and PHQ- Conclusions The incidence of sleep disorders in PD patients in Xinjiang is higher than that in normal subjects, but there is no obvious ethnic difference between Victoria and Han nationalities. The older the PD patients, the longer the duration and the more severe the depression, the worse the sleep status.
