目的探讨脑电图(EEG)与血清S-100β水平对手足口病(HFMD)并发脑炎患儿的早期诊断及预后评估的临床价值。方法以70例确诊的HFMD并发脑炎患儿和70例一般HFMD患儿(普通组)为研究对象。根据HFMD并发脑炎的严重程度将其分为轻度组(32例)、中度组(25例)和重度组(13例);根据患儿临床预后将其分为存活组(58例)及死亡组(12例),采用酶联免疫吸附法检测血清S-100β水平同时检测脑电图,分别比较各组S-100β水平及脑电图的差异以及两者的相关性。结果 HFMD并发脑炎患儿EEG异常程度明显高于普通组(P<0.05);HFMD并发脑炎的严重程度与EEG的异常程度呈显著正相关(r=-0.584,P=0.000),同时死亡组EEG的异常程度也明显高于存活组(P<0.05);HFMD并发脑炎组患儿血清S-100β水平明显高于普通组(P<0.01);轻度组S-100β水平低于中度组和重度组,同时中度组S-100β水平也显著低于重度组(P<0.05);而死亡组S-100β水平显著高于存活组(P<0.05)。相关性分析显示,EEG异常程度与血清S-100β水平呈正相关(r=0.537,P=0.000)。结论 HFMD并发脑炎患儿EEG异常程度与血清S-100β水平呈现较好的相关性,EEG异常程度越高,血清S-100β水平也越高,预后也越差,两者结合可作为HFMD并发脑炎的早期诊断及预测其病情严重程度和预后的有效指标。
Objective To investigate the clinical value of EEG and serum S-100β levels in the early diagnosis and prognosis of HFMD complicated with encephalitis in children. Methods 70 children with HFMD complicated by encephalitis and 70 children with general HFMD (normal group) were enrolled in this study. According to the severity of HFMD complicated with encephalitis, the patients were divided into mild group (32 cases), moderate group (25 cases) and severe group (13 cases) And death group (12 cases). The levels of S-100β in serum were detected by enzyme-linked immunosorbent assay (ELISA). The levels of S-100β and EEG were compared between the two groups. Results The abnormal EEG in children with HFMD complicating encephalitis was significantly higher than that in the normal group (P <0.05). The severity of HF complicated with encephalitis was positively correlated with the abnormal EEG (r = -0.584, P = 0.000) The level of S-100β in children with HFMD complicated by encephalitis group was significantly higher than that in normal group (P <0.01), while the level of S-100β in mild group was lower than that in survivors (P <0.05) (P <0.05). The level of S-100β in the death group was significantly higher than that in the survival group (P <0.05). Correlation analysis showed that there was a positive correlation between EEG abnormalities and serum S-100β (r = 0.537, P = 0.000). Conclusion EEG abnormalities in children with HFMD complicated with encephalitis have a good correlation with serum S-100β levels. The higher the abnormality of EEG, the higher the serum S-100β level and the worse the prognosis, the combination of the two can be used as HFMD concurrent Early diagnosis of encephalitis and predict the severity of the disease and prognosis of the effective indicators.
