1996年7月~1998年12月我院为32例60岁以上伴高血压病的食管癌患者施行了手术切除,本文对此进行总结分析,旨在探讨伴高血压病的食管癌老年患者开胸手术治疗的适应证选择及围手术期处理。 1 临床资料 1996-07-1998-12,对152例60岁以上的食管癌患者行癌肿切除,其中术前合并高血压病者共32例(21.1%)。男22例、女10例,年龄60岁~72岁、平均年龄65.2岁,肿瘤位于胸上段4例、胸中段24例、胸下段4例,均行颈部吻合,病理诊断均为鳞状细胞癌,区域有淋巴结转移者11例(34.4%),按国际分期Ⅱa期20例、Ⅲ期12例;而术前并无高血压病者120例(78.9%),男85例、女35例,年龄60岁~76岁、平均年龄65.3岁,肿瘤位于胸上段12例、胸中段88例、胸下段20例,行颈部吻合116例、弓上吻合1例、弓下吻合3例,病理诊断为鳞状细胞癌113例、小细胞未分化癌3例、粘液表皮样腺癌2例、食管平滑肌肉瘤和原位癌各1例,区域有淋巴结转移者52例(43.4%),按国际分期0期1例、Ⅰ期1例、Ⅱa期48例、Ⅱb期8例、Ⅲ期60例、Ⅳ期2例。
From July 1996 to December 1998, 32 cases of esophageal cancer patients with hypertension who were older than 60 years old underwent surgical resection. The tumors were located in 4 cases in the upper thoracic segment, 24 in the mid thoracic segment and 4 in the lower thoracic segment. All patients underwent neck anastomosis. All pathological diagnoses were squamous cells. There were 11 cases (34.4%) with lymph node metastasis in the cancer area, 20 cases according to international stage IIa and 12 cases in stage III; 120 cases (78.9%) without hypertension before operation, 85 cases for male and 35 cases for female , aged 60-76 years, mean age 65.3 years, tumors located in the upper thoracic segment in 12 cases, 88 in the mid-thoracic segment, 20 in the lower thoracic segment, 116 neck anastomosis, 1 bow anastomosis, 3 anastomosis, pathology There were 113 cases of squamous cell carcinoma, 3 cases of small cell undifferentiated carcinoma, 2 cases of mucoepidermoid adenocarcinoma, 1 case of esophageal leiomyosarcoma, and 1 case of carcinoma in situ. There were 52 cases (43.4%) with regional lymph node metastases. There were 1 case in stage 0, 1 in stage I, 48 in stage IIa, 8 in stage IIb, 60 in stage III, and 2 in stage IV.
