目的探讨高同型半胱氨酸血症与2型糖尿病周围神经病变及胰岛素抵抗的关系。方法检测103例2型糖尿病患者及25例对照组血清同型半胱氨酸(Hcy)水平及血糖、糖化血红蛋白、血脂、胰岛素、胰岛素抵抗指数(Hom a-IR值)等指标。结果与对照组及单纯2型糖尿病组相比,2型糖尿病周围神经病变组血清Hcy水平、高Hcy血症发生率均显著升高(P<0.05),伴高Hcy血症的2型糖尿病患者Hom a-IR值较不伴高Hcy血症的2型糖尿病患者显著升高(P<0.05)。结论高Hcy血症与2型糖尿病并发周围神经病变有关,而2型糖尿病患者高Hcy血症与胰岛素抵抗相关。
Objective To investigate the relationship between hyperhomocysteinemia and peripheral neuropathy and insulin resistance in type 2 diabetic patients. Methods Serum homocysteine (Hcy) level, blood glucose, glycosylated hemoglobin, lipids, insulin and Hom a-IR were detected in 103 type 2 diabetic patients and 25 controls. Results Compared with the control group and the simple type 2 diabetic group, the serum Hcy levels and the incidence of hyperhomocysteinemia in type 2 diabetic peripheral neuropathy group were significantly increased (P <0.05). Patients with type 2 diabetes mellitus Hom a-IR values were significantly higher in patients with type 2 diabetes than those without high Hcy (P <0.05). Conclusions Hyperhomocysteinemia is associated with peripheral neuropathy in type 2 diabetes mellitus, whereas hyperhomocysteinemia in type 2 diabetes is associated with insulin resistance.
