目的 :探讨单纯性肥胖、IGT患者血清空腹游离脂肪酸、C -反应蛋白 (CRP)水平的变化及其与胰岛素抵抗的关系。方法 :采用免疫比浊法分别测定正常人、单纯性肥胖和IGT患者的空腹血清CRP水平 ;同时测定体重指数 (BMI)和腰臀比 (WHR)。用减少样本数的MMM分别测定正常人、单纯性肥胖和IGT患者的胰岛素敏感指数 (SI)。结果 :正常组的SI明显高于单纯性肥胖组和IGT组 (P <0 0 1 ) ;单纯性肥胖、IGT组患者空腹血清空腹胰岛素 (FINS)水平较正常组增高 ,差异有显著性 (P <0 0 5 ) ;IGT组患者FBG、HbA1c较正常组和肥胖组增高 (P <0 0 5 ) ;单纯肥胖组和IGT组CRP高于正常组 (P <0 0 5 ) ,单纯性肥胖组和IGT组之间无明显差异 (P >0 0 5 ) ;相关分析显示CRP与SI呈负相关 (r=-0 60 3 ,P =0 0 0 1 ) ,与BMI(r=0 666,P =0 0 0 0 )和WHR呈正相关 (r=0 3 90 ,P =0 0 40 )。结论 :单纯肥胖和IGT患者CRP水平高于正常人 ,与SI呈负相关 ,与WHR、BMI呈正相关 ,提示单纯肥胖和IGT患者CRP增高与胰岛素抵抗有相关
Objective: To investigate the changes of serum free fatty acids and C - reactive protein (CRP) level in serum and insulin resistance in simple obesity and IGT patients. Methods: Fasting serum CRP levels were determined by immunoturbidimetry in normal subjects, simple obesity patients and IGT patients. Body mass index (BMI) and waist-hip ratio (WHR) were also measured. MMM was used to determine the insulin sensitivity index (SI) of normal subjects, simple obesity patients and IGT patients respectively. Results: The SI of the normal group was significantly higher than that of the simple obesity group and the IGT group (P <0.01). The fasting serum insulin (FINS) level of the IGT group was higher than that of the normal group (P <0.01) (P <0.05). The levels of FBG and HbA1c in IGT group were higher than those in normal group and obesity group (P <0 05). CRP in simple obesity group and IGT group were higher than those in normal group (P <0.05), simple obesity group There was no significant difference between IGT group and IGT group (P> 0.05). Correlation analysis showed that CRP was negatively correlated with SI (r = -0603, P = 0 0 01) = 0 0 0 0) and WHR were positively correlated (r = 0 3 90, P = 0 0 40). CONCLUSIONS: CRP levels in simple obesity and IGT patients are higher than those in normal controls, negatively correlated with SI and positively correlated with WHR and BMI, suggesting that elevated CRP in simple obesity and IGT patients is associated with insulin resistance
