[病例]男,47岁。因胸闷、气短4天,心前区疼痛9小时入院。4天前于劳累后出现胸闷、气短、腹胀、少尿,日尿量400ml。于入院当日清晨6时睡眠中突然出现胸骨后疼痛,并向左肩、左上臂放散,同时伴有大汗、濒死感。原发性扩张型心肌病病史4个月。查体:血压12/0kPa,口唇无发绀,颈静脉轻度怒张.两肺呼吸音粗,未闻及干湿性罗音,心尖搏动位于第六肋间左锁骨中线外1.0cm处,叩诊心界向两侧扩大,心率180次/min,
