产程中胎儿血气和 pH 值的直接信息只能从胎儿头皮的微循环中获取。如头皮血流无改变,末梢血气和 pH 值将反应颈动脉值;但如头皮血流减少,局部皮肤代谢会使其出现很大差别。作者利用多普勒转变的单色激光,经头皮微血管床内移动的红细胞散播回收来确定血流指数。本文报告15例头皮血流记录並注意其产程中的总趋势。作者使用低功率氦氖激光(λ=632.8nm)通过光导纤维及粘合在胎儿头皮上的探头进行测定,每分钟定量分析一次並按照 Stern 等(1977年)的公式计算血流指数。出生后1分钟取脐血並于5分钟内测定 pH 值及血气。15例中初产6例,经产9例,均为单胎。平均孕龄40周。大多因妊娠高血压或过期妊娠引产和胎儿监护。激光多普勒血流(LDF)的测定在宫口平均开大5 cm(3~8cm)时开始,平均记录时
Fetal blood gas and pH in the labor process can only be obtained from the microcirculation of fetal scalp. No change in the scalp blood flow, peripheral blood gas and pH values will respond to the carotid artery value; but as the scalp blood flow decreases, the local skin metabolism will make a big difference. The authors used a monochromatic laser that was converted by Doppler to determine the index of blood flow by disseminating red blood cells that migrated through the scalp microvascular bed. This article reports 15 cases of scalp blood flow records and pay attention to the general trend in the labor process. The authors measured low-power He-Ne laser (λ = 632.8 nm) through optical fibers and probes bonded to the fetal scalp. Quantitative analysis was performed every minute and the blood flow index was calculated according to the formula of Stern et al. (1977). Umbilical cord blood was taken 1 minute after birth and the pH value and blood gas were measured within 5 minutes. In 15 cases, 6 cases were primiparous and 9 cases were produced, all of whom were singletons. Average gestational age 40 weeks.
