目的 探讨脉冲组织多普勒成像技术(PWGTDI)评估蒽环类药物对乳腺恶性肿瘤患者左心室收缩功能影响的应用价值.方法 利用PWGTDI获取左室心尖二腔心切面、心尖四腔心切面、心尖左室长轴切面测量二尖瓣环运动速度(Vs),对心尖二腔观的前壁(LVAW)、下壁(LVIW),心尖四腔观的侧壁(LVLW)、后间隔(LVPS),心尖长轴观的前间隔(LVAS)、后壁(LVPW)的二尖瓣环6个节段收缩期峰值速度.结果 二尖瓣环Vs对照组与蒽环类药物化疗6个周期后比较,蒽环类药物化疗6个周期后患者低于对照组,差异均有统计学意义,(P<001).结论 脉冲组织多普勒成像技术可以较早期、更敏感、无创评价蒽环类药物对肿瘤患者左心室功能的损害.利用PWGTDI测定二尖瓣环Vs诊断早期左心室收缩功能受损是敏感和可靠的.
Objective To evaluate the value of pulsed Tissue Doppler Imaging (PWGTDI) in assessing the effect of anthracycline on the left ventricular systolic function in patients with malignant breast neoplasms.Methods The left ventricle and the left ventricular systolic function were obtained by PWGTDI, (LVs), inferior wall (LVIW), apical four-chamber wall (LVLW), posterior septum (LVPS) , Anterior long axis (LVAS) and posterior wall (LVPW) peak velocities of 6 segments of mitral annulus were measured.Results Compared with 6 cycles of mitral annular Vs control group and anthracycline chemotherapy (P <0.001) .Conclusion Pulsed Tissue Doppler imaging can be used in the early, more sensitive and noninvasive evaluation of anthracycline drugs in patients with anthracycline chemotherapy after six cycles of chemotherapy, the difference was statistically significant Impairment of Left Ventricular Function in Tumor Patients. The use of PWGTDI for mitral annular Vs in the diagnosis of early left ventricular systolic dysfunction is sensitive and reliable.
