目的:探讨在BPH和PCa中MVD、微血管的相关指标及PCNA和MSCT多期扫描强化特征的相关性。方法:对经病理确诊的35例BPH和27例PCa患者术前均行MSCT扫描,绘制BPH和PCa时间-密度曲线(T-DC)并计算曲线升段的斜率(S);所有标本行CD34、PCNA的免疫组化染色。结果:时间-密度曲线分为3种类型,3种曲线在两组病例分布的差异有统计学意义,P<0.01。BPH和PCa组织中MVD分别为11.00±4.14和32.11±13.19,BPH微血管平均直径等指标比PCa的相应指标要大,P<0.01。PCNA在PCa中的表达高于在BPH中的表达,P<0.01。斜率(S)与MVD有相关性,P<0.01;在PCa中,斜率(S)与PCa的Gleason分级、肿瘤分化程度有相关性,P<0.05。PCNA的表达仅与PCa的Gleason分级、肿瘤分化程度分级和临床分期均有相关性,P<0.05。结论:MSCT多期扫描强化特征能够显示BPH、PCa的血供情况,在一定程度上反映增生及癌组织中微血管的生成以及推测其生物学行为。
OBJECTIVE: To investigate the correlation between MVD and microvessel in BPH and PCa and the multi-phase scan enhanced features of PCNA and MSCT. Methods: 35 cases of pathologically diagnosed BPH and 27 cases of PCa patients were preoperative MSCT scan, draw BPH and PCa time-density curve (T-DC) and calculate the slope of the rising curve (S); all specimens of CD34 , Immunohistochemical staining of PCNA. Results: The time-density curves were divided into three types. There were significant differences in the distribution of the three kinds of curves between the two groups (P <0.01). The MVD in BPH and PCa tissues were 11.00 ± 4.14 and 32.11 ± 13.19, respectively. The mean diameter of microvessels in BPH was higher than that in PCa (P <0.01). The expression of PCNA in PCa was higher than that in BPH, P <0.01. The slope (S) was correlated with MVD, P <0.01. In PCa, the slope (S) was correlated with the Gleason grade of PCa and the degree of tumor differentiation (P <0.05). PCNA expression was only associated with PCa Gleason grade, grade of tumor differentiation and clinical stage were related, P <0.05. Conclusion: MSCT multi-phase scanning enhanced features can show the blood supply of BPH and PCa to some extent, reflecting the formation of microvessels in hyperplasia and cancerous tissues and speculating on their biological behavior.
