目的评价踝肱指数(Ankle brachial pressure index,ABI)和颈动脉内膜-中膜厚度(carotid intimae-medial thickness,IMT)在2型糖尿病大血管病变中的临床意义,探讨ABI联合颈动脉IMT测量对2型糖尿病大血管病变的预测价值。方法选取2型糖尿病患者211例,测量ABI和颈动脉IMT。根据ABI和颈动脉IMT的测量结果,评估患者罹患心脑血管疾病的患病危险比。结果①ABI降低,冠心病(CHD)、脑血管病(CVD)和颈动脉粥样硬化(CAS)患病率显著增加。②颈动脉IMT增厚,CHD、CVD和周围动脉疾病(peripheral artery disease,PAD)患病率显著增加。③随着ABI/IMT分值的增加,CHD和CVD的患病率显著增加。结论 ABI和颈动脉IMT均可作为动脉粥样硬化的预测指标,两者联合测量较单独测量对2型糖尿病大血管病变的检出率更高。
Objective To evaluate the clinical significance of Ankle brachial pressure index (ABI) and carotid intimae-medial thickness (IMT) in type 2 diabetic macroangiopathy. To explore the relationship between ABI and carotid IMT Predict value of type 2 diabetes macroangiopathy. Methods 211 patients with type 2 diabetes mellitus were selected to measure ABI and carotid IMT. Based on the ABI and carotid IMT measurements, patients were assessed for their risk of developing cardiovascular and cerebrovascular diseases. Results ①ABI decreased, the prevalence of coronary heart disease (CHD), cerebrovascular disease (CVD) and carotid atherosclerosis (CAS) increased significantly. ② The carotid artery IMT thickening, CHD, CVD and peripheral artery disease (PAD) prevalence increased significantly. ③ With the increase of ABI / IMT scores, the prevalence of CHD and CVD increased significantly. Conclusions Both ABI and carotid IMT can be used as predictors of atherosclerosis. The combination of ABI and carotid IMT has a higher detection rate of type 2 diabetes mellitus than that of single measurement.
