本文对234例(男99例,女135例)糖尿病门诊患者进行了详细的神经系统病史询问及检查,其中有140例作了外展拇短肌肌电图以及正中神经和胫后神经的感觉和运动传导速度(SCV和MCV)测定。确诊为糖尿病性神经病变者158例(67.5%)。发现本组糖尿病性神经病变的发生与糖尿病发病年龄无关,但糖尿病病程在5年以上的患者并发神经病变的例数较5年以下者为多。作者提出糖尿病性神经病变的早期诊断指标是:①神经传导速度减慢,尤其是胫后神经的感觉传导速度减慢;②下肢音叉震动觉减弱或消失;③膝、踝反射减弱或消失;④下肢麻木,无力,疼痛。
