目的检测大肠息肉、癌及相应正常大肠粘膜组织CD44基因mRNA表达,探讨该基因表达在大肠肿瘤进展转移中的意义以及癌组织上皮源性亚型与造血源性亚型比值(E/H)的临床意义。方法应用逆转录、多聚合酶链反应和Southern杂交技术,检测26例大肠癌患者癌组织、正常大肠粘膜组织和7例大肠息肉CD44基因mRNA表达。结果息肉、癌、转移淋巴结中上皮源性亚型及含V6、V10外显子的变异型均明显表达,而正常粘膜组织中表达不明显。Dukes分期A、B期癌E/H为14±10,C、D期癌E/H为14±09;癌浸润肌层者E/H为16±10,浸润全层者E/H为13±09;有肝或淋巴结转移者E/H为14±10,无转移者为14±10,未发现E/H与肿瘤分期、有无转移、浸润深度有关(t检验,P>005)。结论CD44mRNA包括含有V6、V10外显子的变异型在大肠肿瘤发生发展的早期已有表达,变异型的表达在大肠肿瘤发生发展中具有重要意义。E/H值的临床价值需进一步探讨。
Objective To detect the mRNA expression of CD44 in colorectal polyps, carcinoma and corresponding normal colorectal mucosa, and to explore the significance of the gene expression in the progression and metastasis of colorectal tumor and the ratio of epithelial to hematopoietic subtype (E / H) Clinical significance. Methods Reverse transcription, polymerase chain reaction and Southern blotting were used to detect the mRNA expression of CD44 in 26 cases of colorectal cancer, normal colorectal mucosa and 7 cases of colorectal polyps. Results The epithelial-derived subtypes and the variants containing V6 and V10 exons in polyps, cancers and metastatic lymph nodes were all significantly expressed, but not in normal mucosa tissues. The E / H of Dukes stage A and B cancer was 14 ± 10, and the E / H of C and D stage carcinoma was 14 ± 09. The E / H of cancer infiltrating myometrium was 16 ± 1 0, E / H was 13 ± 09 in infiltrating the whole layer; E / H was 14 ± 10 in patients with liver or lymph node metastasis, 14 ± 10 in none metastasis Found E / H and tumor stage, with or without metastasis, depth of invasion (t test, P> 0 05). Conclusion CD44 mRNA, including V6 and V10 exon variants, have been expressed in the early stage of colorectal neoplasms. The aberrant expression is of great significance in the development of colorectal tumors. The clinical value of E / H value needs to be further explored.
