目的对颅脑动脉瘤夹闭术后脑血管痉挛患者感染病原菌分布及对患者血清球蛋白表达的影响进行研究,为临床诊治提供参考依据。方法对医院2010年1月-2014年11月期间诊治210例颅脑动脉瘤夹闭术后脑血管痉挛感染患者为感染组,以医院同期健康体检者210名为对照组,颅脑动脉瘤夹闭术后脑血管痉挛未感染患者600例设为未感染组,分析病原菌分布及临床指标。结果感染组患者中共分离210株病原菌,以革兰阳性菌为主,共107株占51.0%;感染患者血清球蛋白、白蛋白及血清总蛋白水平明显低于对照组和未感染组患者,而血清JAK1、JAK3和STAT1及STAT2蛋白水平明显高于对照组和未感染组患者,差异均有统计学意义(P<0.05)。结论颅脑动脉瘤夹闭术后脑血管痉挛患者容易受到病原菌感染,且感染引起患者血清白蛋白和球蛋白水平降低和JAK/STAT通路蛋白表达的升高。
Objective To study the distribution of pathogenic bacteria in patients with cerebral vasospasm after cerebral aneurysm clipping and the effect on the expression of serum globulin in patients with clipped aneurysms and provide a reference for clinical diagnosis and treatment. Methods A total of 210 patients with cerebrovascular spasm infection after occlusion of craniocerebral aneurysm during the period from January 2010 to November 2014 were selected as the infected group. Totally 210 healthy subjects were recruited as the control group, Six hundred patients without cerebrovascular spasm after closed surgery were enrolled as uninfected group, and the distribution of pathogens and clinical indexes were analyzed. Results A total of 210 pathogenic bacteria were isolated from patients with infection, of which 107 (51.0%) were gram-positive bacteria. Serum globulin, albumin and serum total protein in patients with infection were significantly lower than those in control and non-infected patients Serum JAK1, JAK3, STAT1 and STAT2 protein levels were significantly higher than the control group and non-infected patients, the difference was statistically significant (P <0.05). Conclusions Patients with cerebrovascular spasm after craniocerebral aneurysm clipping are susceptible to pathogen infection. Infection causes the decrease of serum albumin and globin level and the increase of JAK / STAT pathway protein.
