目的:观察中药内服联合外洗治疗早期老年糖尿病患者的临床效果。方法:选取2013年7月~2014年7月于我院接受治疗的100例早期老年糖尿病足患者作为研究对象,按照随机对照法将其分为研究组与对照组两组,每组50例。对照组仅接受基础治疗,研究组则在此基础上加用中药内服外治治疗。观察两组患者临床症状积分的变化情况,记录治疗前后两组患者踝肱指数、腓总神经感觉传导速度的变化,观察两组患者的临床治疗效果。结果:研究组治愈26例,有效15例,好转6例,有效率为94.00%,明显高于对照组(72.00%),组间对比差异有统计学意义(P<0.05);治疗后,研究组下肢溃疡、疼痛、跛行、冷感症状积分分别降低至(0.21±0.14)分、(1.50±0.61)分、(1.42±0.64)分、(1.49±0.61)分,均明显低于对照组(P<0.05);治疗后,研究组腓总神经感觉传导速度提升至(29.72±5.81)m/s,踝肱指数上升至(0.88±0.27)%,均高于对照组(P<0.05)。结论:在早期老年糖尿病足患者的临床治疗中,提倡采用中药内服联合外洗治疗方案,不仅可优化临床疗效,同时可提升其足功能,改善患者临床症状,值得推广。
Objective: To observe the clinical effect of combined oral administration and external washing of traditional Chinese medicine in the treatment of early-stage diabetic patients. Methods: A total of 100 elderly patients with early-stage diabetic foot treated in our hospital from July 2013 to July 2014 were selected as study subjects and divided into study group and control group according to randomized control group, 50 cases in each group. The control group received only basic treatment, the research group was based on the use of traditional Chinese medicine and external treatment. The changes of clinical symptom score in both groups were observed. The changes of ankle-brachial index and peroneal nerve sensory conduction velocity in both groups were recorded before and after treatment. The clinical effects of the two groups were observed. Results: The study group cured 26 cases, effective in 15 cases, improved in 6 cases, the effective rate was 94.00%, significantly higher than the control group (72.00%), the difference between the groups was statistically significant (P <0.05); after treatment, The scores of ulcer, pain, claudication and cold sensation decreased to (0.21 ± 0.14) points, (1.50 ± 0.61) points, (1.42 ± 0.64) points and (1.49 ± 0.61) points in the lower limbs, P <0.05). After the treatment, the sensory velocity of the common peroneal nerve increased to (29.72 ± 5.81) m / s and the ankle brachial index increased to (0.88 ± 0.27)% in the study group, which were all higher than those in the control group (P <0.05). Conclusion: In the early clinical treatment of elderly patients with diabetic foot, promoting the use of Chinese medicine combined with external wash treatment program, not only to optimize the clinical efficacy, while improving its foot function and improve clinical symptoms, it is worth promoting.
