目的通过对短时受精结合早期补救卵细胞质内单精子注射(ICSI)与部分ICSI两种授精方式卵子利用率、胚胎质量和妊娠结局的比较,为前一周期常规体外受精-胚胎移植(IVF-ET)受精率低下(PFF)患者寻找到最适合的授精方法。方法选取2008年12月~2013年8月该院生殖医学中心前次常规IVF助孕治疗发生PFF的第2周期,且取卵日处理前后精液参数正常患者41例为研究对象,根据授精方式不同随机分为两组,短时受精组(短时组)19例,部分ICSI组(Half组)22例。比较两组间受精、卵裂情况、胚胎质量、着床率、妊娠率、成熟卵子利用率及受精方式为IVF的成熟卵子利用率。结果短时组的双原核受精率,多精率,成熟卵子利用率及受精方式为IVF的成熟卵子利用率分别为59.53%,8.83%,56.98%和18.44%,明显高于Half组的41.70%,2.47%,42.41%和8.93%。结论对于前次常规IVF受精率低下患者,短时受精结合早期补救ICSI与部分ICSI授精方式的胚胎质量及临床结局无差异,但前者有更高的成熟卵子利用率和更多可利用的胚胎,特别是授精方式为IVF的胚胎。
Objective To compare the effects of short-term fertilization and early recovery oocyte incytoplasmic sperm injection (ICSI) and partial ICSI fertilization on egg utilization, embryo quality and pregnancy outcome, to evaluate the effect of IVF- ET) patients with low fertilization (PFF) to find the most suitable method of insemination. Methods From December 2008 to August 2013, the second cycle of IVF pregnancy-assisted treatment of PFF was conducted at the Reproductive Medicine Center of this hospital from January 2008 to August 2013, and 41 patients with normal sperm parameters before and after ovulation day treatment were selected according to different insemination methods Nineteen patients were randomly divided into two groups: short-term fertilization group (short-term group), partial ICSI group (n = 22). Fertilization, cleavage, embryo quality, implantation rate, pregnancy rate, mature egg utilization rate and fertilization rate of IVF were compared between the two groups. Results The digestible fertilization rate, multiple fertilization rate, mature egg utilization rate and fertilization rate of short fertile group were 59.53%, 8.83%, 56.98% and 18.44%, respectively, which were significantly higher than those of the half group (41.70% , 2.47%, 42.41% and 8.93% respectively. Conclusions There was no difference in embryo quality and clinical outcome between short-term fertilization combined with early salvage ICSI and some ICSI fertilization methods in patients with previous IVF fertilization, but the former had higher mature egg utilization rate and more available embryos, In particular, IVF embryos are inseminated.
