目的 观察组织型纤溶酶原激活剂 (tissueplasminogenactivator,t PA)治疗儿童人工晶体植入术后晶体前机化膜的合理疗效。方法 第 1组 1 4只眼 ,在术后 4~ 7天前房内形成纤维蛋白膜时 ,于前房内注射 5 μgt PA。第 2组 4只眼 ,术后形成较厚的机化膜 ,在术后 5~ 9天时先用YAG激光切开纤维膜 ,再向前房内注射 5 μgt PA。 结果 第 1组 1 4只眼内的纤维蛋白膜 2 4小时内全部溶解吸收 ,无前房出血。第 2组患者中 ,3只眼瞳孔区的机化膜于 4~ 2 4小时内全部吸收 ,1只眼 2 4小时后仍有部分残留 ,而再次行前房内注射 5 μgt PA后于 1 2小时内吸收。无前房出血。 结论 在术后 4~ 9天时 ,前房内炎症稳定的情况下 ,小剂量t PA前房内注射治疗晶体前纤维蛋白性膜十分有效。YAG激光联合治疗的作用更加明显。
Objective To investigate the reasonable therapeutic effect of tissueplasminogen activator (t PA) on the premature lens in children after intraocular lens implantation. Methods In group 1, 14 eyes were injected with 5 μg of PA in the anterior chamber after the formation of fibrin membrane in the room from 4 to 7 days after operation. The second group of 4 eyes, thicker after surgery to form a mechanical film, 5-9 days after the first YAG laser incision fiber membrane, and then into the anterior chamber injection 5 μgt PA. Results In Group 1, fibrin membranes in 14 eyes were completely dissolved and absorbed within 4 hours without any bleeding in the anterior chamber. Group 2 patients, 3 eyes of the pupil area of the mechanized membrane in 4 ~ 2 4 hours all absorbed, 1 eye 2 4 hours after there is still some residual, and again before the line after injection of 5 μ PA PA Absorbed within 2 hours. No anterior chamber bleeding. Conclusions In the case of stable inflammation in the anterior chamber at 4 to 9 days after operation, a small dose of t PA injected into the anterior chamber of the lens is very effective in the treatment of crystalline fibrin membranes. YAG laser combination therapy is more obvious.
