目的 :观察低分子肝素在不稳定心绞痛 (UA)中的治疗作用 ,并探讨其可能机制 ,为临床UA治疗提供理论依据。方法 :将 6 5例患者随机分为二组 :A组 ,34例 ,常规抗心绞痛 (AP)治疗 [单用或联合用 β 受体阻滞剂、钙通道阻滞剂、静脉滴注硝酸甘油 (NG) ];B组 ,31例 ,常规抗AP治疗 +低分子肝素 (LMWH)皮下注射 ,一日二次 ,连续10d ,观察治疗前 10d临床情况及LMWH治疗前后血液流变学情况。结果 :治疗 10d内LMWH组AP发作频率 ,NG用量 ,2 4h缺血时间明显减少 ;LMWH治疗后血液流变学各指标明显好转。结论 :LMWH可通过降低血液粘度 ,改善微循环 ,改善UA的临床症状 ,出血副作用小
Objective: To observe the therapeutic effect of low molecular weight heparin in unstable angina pectoris (UA) and to explore its possible mechanism to provide a theoretical basis for clinical UA treatment. Methods: Sixty-five patients were randomly divided into two groups: group A, 34 patients treated with conventional anti-angina pectoris (AP) alone or in combination with beta blockers, calcium channel blockers, intravenous nitroglycerin (NG)]. In group B, 31 patients were given anti-AP therapy and LMWH subcutaneously twice a day for 10 days. The clinical situation and the hemorheology before and after LMWH treatment were observed 10 days before treatment. Results: The frequency of AP seizure, the dosage of NG and the ischemic time of 24 h were significantly decreased in LMWH group within 10 days after treatment. The indices of hemorrheology in LMWH group were significantly improved after treatment. Conclusion: LMWH can reduce the blood viscosity, improve microcirculation, improve the clinical symptoms of UA, bleeding side effects
