目的探讨2型糖尿病患者凝血状态。方法选取2014年1月至2015年6月在沈阳市浑南区中心医院就诊的糖尿病患者147例,其中单纯糖尿病患者49例为研究对象,为单纯糖尿病组。糖尿病合并高血压患者47例,为糖尿病合并高血压组,糖尿病合并冠状动脉粥样硬化性心脏病患者51例,为糖尿病合并冠心病组。另外选取健康体检者50例为对照组。观察4组活化部分凝血活酶时间(APTT)、血浆凝血酶原时间(PT)、血浆纤维蛋白原(FB)、血浆凝血酶时间(TT),并评价凝血功能。结果单纯糖尿病组、糖尿病合并高血压组、糖尿病合并冠心病组PT、APTT均低于对照组,FB高于对照组,差异均有统计学意义(均P<0.05);糖尿病合并高血压组、糖尿病合并冠心病组FB高于单纯糖尿病组,TT低于单纯糖尿病组,差异均有统计学意义(均P<0.05);糖尿病合并冠心病组FB高于糖尿病合并高血压组,TT低于糖尿病合并高血压组,差异均有统计学意义(均P<0.05)。结论糖尿病患者存在凝血功能异常,处于血栓前状态,关注患者的凝血功能可以对疾病进行及早干预。
Objective To investigate the coagulation state in type 2 diabetic patients. Methods A total of 147 diabetes patients from January 2014 to June 2015 in Hunnan District Central Hospital of Shenyang were enrolled. Among them, 49 patients with simple diabetes mellitus were selected as the study group. Forty-seven patients with diabetes mellitus and hypertension were diabetes mellitus with hypertension, 51 patients with diabetes mellitus and coronary atherosclerotic heart disease, and were diabetic with coronary heart disease. In addition, 50 healthy people were selected as the control group. The activated partial thromboplastin time (APTT), plasma prothrombin time (PT), plasma fibrinogen (FB) and plasma thrombin time (TT) were observed in 4 groups. The coagulation function was also evaluated. Results Compared with control group, the levels of PT and APTT in diabetic mellitus patients with diabetes mellitus, hypertension with diabetes mellitus and coronary heart disease with diabetes mellitus were significantly lower than those in control group (all P <0.05). In diabetic patients with hypertension, Diabetes with coronary heart disease group FB higher than simple diabetic group, TT was lower than simple diabetic group, the difference was statistically significant (all P <0.05); diabetes mellitus coronary heart disease group FB was higher than that of diabetic patients with hypertension, TT was lower than diabetes Combined hypertension group, the difference was statistically significant (P <0.05). Conclusion There is coagulation abnormalities in patients with diabetes mellitus, in the pre-thrombosis state, focusing on the patient’s blood coagulation can make an early intervention in the disease.
