目的了解四川省绵阳市男男性行为者(men who have sex with men,MSM)首次与近6个月同性性行为保护性的关联性,分析其内在一致性的影响因素。方法 2015年3~10月,以四川省绵阳市MSM为对象,用滚雪球抽样法,在社会组织办公室、茶园、卡拉OK厅等场所,进行调查对象招募和自填式匿名行为学调查及血清学检测。结果共调查有效问卷1 233份,首次同性性行为年龄平均(20.4±6.4)岁,有保护性42.40%,近6个月同性性行为有保护性82.99%。首次同性性行为、近6个月同性性行为保护性相关联(P=0.001),均有保护性87.47%(正性一致),均无保护性20.07%(负性一致),差异有统计学意义(P=0.001)。首次性行为、近6个月性行为保护性正性一致艾滋病病毒(human immunodeficiency virus,HIV)感染率4.51%,低于负性一致的11.38%(P<0.05)。多因素分析结果显示:在婚/离异丧偶、成为MSM时间>5年、近1周肛交次数>5次、有压力感受、压力来自人际关系、压力来自家庭是首次性行为、近6个月性行为保护性负性一致的促进因素,文化程度大专及以上是保护因素(P均<0.05)。结论 MSM首次性行为、近6个月同性性行为的保护性呈正相关,在婚/离异丧偶、成为MSM时间>5年、近1周肛交次数>5次、有压力感受、压力来自人际关系或家庭等是降低性行为保护一致性的影响因素,MSM性行为保护负性一致组HIV感染率显著高于正性一致组。
Objective To understand the relationship between men who have sex with men (MSM) and the protection of homosexual behaviors for the first time in Mianyang, Sichuan Province, and to analyze the influential factors of their inherent consistency. Methods From March to October 2015, MSM in Mianyang City of Sichuan Province was used as the research object, and the snowball sampling method was used to investigate the recruiting and self-contained anonymous behaviors in social organizations offices, tea gardens and karaoke halls. Detection. Results A total of 1 233 valid questionnaires were investigated. The mean age of first-time same-sex sexual behavior was (20.4 ± 6.4) years old, with a protection of 42.40%. The homosexual behavior was 82.99% in the past 6 months. First-time homosexual behavior, homosexual behavior protection for the past 6 months (P = 0.001), were protective 87.47% (positive agreement), were not protective 20.07% (negative consistency), the difference was statistically significant Significance (P = 0.001). In the first sexual intercourse, HIV infection rate was 4.51% for the first 6 months of sexual intercourse, which was lower than 11.38% (P <0.05) of the negative consensus. Multivariate analysis showed that: in marriage / divorced widowhood, MSM time> 5 years, nearly 1 week anal intercourse> 5 times, have feelings of stress, pressure from interpersonal relationships, the pressure from the family is the first sexual behavior, nearly 6 months of sexual Negative consistent behavioral protective factors, college education and above are protective factors (all P <0.05). CONCLUSIONS: The first sexual behavior of MSM was positively related to the protectiveness of same-sex sexual behavior in the past 6 months. It was stressful when the MSM was> 5 years and the number of anal intercourse was> 5 times in the past 1 week. The pressure was from the interpersonal relationship or Family and other factors to reduce the consistency of sexual behavior protection, MSM sexual behavior protection negative consistent HIV infection was significantly higher than the positive consistent group.
