目的对比分析EGFR与Her-2蛋白的表达及基因扩增情况。方法应用免疫组化(IHC)和荧光原位杂交(FISH)技术对89例肺腺癌和335例乳腺癌分别进行EGFR和Her-2蛋白表达及基因扩增检测。结果 89例肺腺癌:原位腺癌EGFR的蛋白表达和基因扩增病例明显少于以贴壁生长方式为主和以非贴壁生长方式为主的浸润性腺癌(P<0.05),EGFR蛋白表达与基因扩增之间呈显著的正相关(P<0.05)。335例乳腺癌:IHC法Her-2蛋白()109例(32.54%),与FISH法一致率为90.83%(99/109);Her-2()96例(28.66%),与FISH法一致率为53.13%(51/96);Her-2(+)55例(16.42%),与FISH法一致率为10.91%(6/55)。Her-2蛋白表达与扩增之间呈显著正相关(P<0.01),并且156例Her-2基因扩增病例中,全部为浸润性导管癌。结论利用IHC方法对蛋白表达进行初步筛选,进而行FISH基因扩增检测,可为临床进一步筛选病例、选择治疗方案、评估预后提供可靠的依据。
Objective To compare and analyze the expression of EGFR and Her-2 protein and the gene amplification. Methods Immunohistochemistry (IHC) and fluorescence in situ hybridization (FISH) were used to detect the expression of EGFR and Her-2 in 89 cases of lung adenocarcinoma and 335 cases of breast cancer. Results 89 cases of lung adenocarcinoma: in situ adenocarcinoma of EGFR protein expression and gene amplification cases were significantly less than the adherent growth mode and non-adherent growth-based invasive adenocarcinoma (P <0.05), EGFR There was a significant positive correlation between protein expression and gene amplification (P <0.05). Among 335 cases of breast cancer, 109 cases (32.54%) of Her-2 protein () in IHC method were found to be 90.83% (99/109) consistent with FISH and 96 (28.66%) were Her-2 The agreement rate was 53.13% (51/96). Her-2 (+) 55 cases (16.42%) were consistent with FISH. The coincidence rate was 10.91% (6/55). There was a significant positive correlation between Her-2 protein expression and amplification (P <0.01), and 156 cases of Her-2 gene amplification were all invasive ductal carcinoma. Conclusion The preliminary screening of protein expression using IHC method and further detection of FISH gene amplification may provide a reliable basis for further screening cases, selecting treatment options and evaluating prognosis.
