目的研究应用阴道镜和高危型人乳头状瘤病毒(HPV)-DNA检测对宫颈不典型病变筛查的意义,以期提高临床诊治精度,更好地进行后续治疗。方法选取该院2012年4月~2014年7月收治的薄层液基细胞学技术(TCT)检查结果为意义未明不典型鳞状上皮(AS-CUS)的141例患者为研究对象。所有患者均具有一定的接触性阴道出血症状,分别对其进行单独阴道镜检查、高危型HPV-DNA检测和两者联合检查。通过比较检测结果,分析、评价三者的灵敏度及特异度情况。结果联合筛查手段下检测出宫颈不典型病变的精度(包括灵敏度、特异度、预测值)总体明显优于本实验中提及的任何一种单一检测方法,且差异有统计学意义(P<0.05)。结论应用阴道镜检查联合高危型HPV-DNA检测可显著提高临床诊断水平,本组有接触性阴道出血且TCT结果为AS-CUS的实验患者存在宫颈癌变的可能,宜尽早介入治疗。
Objective To study the significance of using colposcopy and high-risk human papillomavirus (HPV) -DNA in the screening of cervical atypical lesions in order to improve the accuracy of clinical diagnosis and treatment and better follow-up treatment. Methods A total of 141 patients with unknown atypical squamous epithelium (AS-CUS) who underwent TLC-based cytology (TCT) in our hospital from April 2012 to July 2014 were selected as the study subjects. All patients have some contact vaginal bleeding symptoms, respectively, for their colposcopy alone, high-risk HPV-DNA test and the combination of both. By comparing the test results, analysis, evaluation of the sensitivity and specificity of the three cases. Results The accuracy (including sensitivity, specificity and predictive value) of detecting cervical atypical lesions under combined screening was significantly better than that of any single detection method mentioned in this study (P < 0.05). Conclusion Colposcopy combined with high-risk HPV-DNA test can significantly improve the level of clinical diagnosis, the group had contact with vaginal bleeding and TCT results AS-CUS experimental cervical cancer patients may exist, should intervention as soon as possible.
