目的:探讨ω-3多不饱和脂肪酸(ω-3PUFA)治疗急性肺损伤(ALI)病人的临床效果及其应用价值。方法:计算机检索PubMed、EMBASE、SCI、Cochrane Library、中国生物医学文献数据库、中文科技期刊全文数据库、中国期刊全文数据库和中华医学会数字化期刊,并辅助手工检索,收集采用ω-3PUFA治疗ALI的随机对照试验(RCT),检索至2012年10月截止。采用Jadad评分评价纳入研究质量,用RevMan 5.1软件进行Meta分析。结果:共纳入6个RCT,各试验间资料均有可比性。经Meta分析结果显示,试验组(ω-3PUFA治疗组)与对照组(常规治疗组)在ALI病人的动脉氧分压(PaO2)、氧合指数(PaO2/FiO2)、入住ICU时间(LOS)和总蛋白(TP)等方面均有显著性差异,而撤离呼吸机时间(LOV)和不良反应则无显著性差异。结论:ω-3PUFA可有效地改善ALI病人的呼吸功能,促进病人呼吸功能的恢复和预后。但其长期疗效和不良反应仍需开展更多高质量的研究。
Objective: To investigate the clinical effect and its application value of ω-3 polyunsaturated fatty acid (ω-3 PUFA) in patients with acute lung injury (ALI). METHODS: PubMed, EMBASE, SCI, Cochrane Library, Chinese Biomedical Literature Database, Chinese Science and Technology Journals Full-text Database, Chinese Journal Full-text Database and Chinese Medical Association Digital Periodicals were searched by computer. Controlled Trial (RCT), searched until October 2012. The quality of the study was included in the Jadad assessment and Meta analysis was performed using RevMan 5.1 software. Results: A total of 6 RCTs were included, and the data of all the trials were comparable. The results of Meta analysis showed that there was no significant difference between the experimental group and the control group on the PaO2, PaO2 / FiO2, LOS, And total protein (TP), etc. There was no significant difference in evacuation ventilator time (LOV) and adverse reactions. CONCLUSION: Omega-3 PUFA can effectively improve the respiratory function of ALI patients and promote the recovery and prognosis of respiratory function. However, its long-term efficacy and adverse reactions still need to carry out more high-quality research.
