1例55岁女性患者因乳腺癌术后淋巴结、骨、肝转移给予多西他赛140 mg静脉滴注。化疗第6天晨出现大量鲜血便,心率120次/min。血常规检查:白细胞计数0.9×10~9/L,血红蛋白103g/L,血小板计数62×10~9/L。立即给予血凝酶2 U静脉注射,氨甲苯酸0.2 g静脉滴注。当日午患者再次出现大量鲜血便,诉心悸、乏力。血常规检查示白细胞计数0.5×10~9/L,血红蛋白75g/L,血小板计数52×10~9/L。给予云南白药1 g+凝血酶冻干粉4000 U+冰生理盐水100 ml,1次/2 h灌肠;生长抑素3 mg静脉滴注持续12 h;输注悬浮红细胞2 U、血小板1 U。电子结肠镜检查见直肠多处溃疡,1处有鲜血渗出。予内镜下止血,患者未再出血。

A large amount of bloody stool appeared on the 6th day of chemotherapy, with a heart rate of 120 beats / min. In the afternoon there was a great deal of bloody stool again, complaining of heart palpitations and weakness. Blood tests showed a white blood cell count 0.5 × 10 ~ 9 / L, hemoglobin 75g / L, platelet count 52 × 10 ~ 9 / L. Given Yunnan Baiyao 1 g + thrombin freeze-dried powder 4000 U + ice saline 100 ml, 1/2 h enema; somatostatin 3 mg intravenous infusion for 12 h; infusion of suspended red blood cells 2 U, 1 U platelet. Electronic colonoscopy see multiple rectal ulcers, one exudation of blood. To endoscopic hemostasis, the patient no longer bleeding.
