患者王××,26岁。1982年10月27日因第一胎孕42周,频繁抽搐伴昏迷2小时入院。孕30周开始双下肢浮肿,半月前浮肿加重并波及全身。入院前2小时下腹阵痛、头昏、视力模糊,随即全身抽搐不省人事。检查:营养发育差,神志不清,全身肌肉强直。双侧瞳孔等大,对光反射迟钝。体温为37.4℃,心率118次。呼吸32次,血压170/110。全身浮肿。宫底剑下3指,右枕前位,宫缩强,胎心音快慢不均,每5秒钟分别为14次、8次、13次。骨盆无明显异常。阴道少量血性分泌物,阴道内触及一肉样软组织,取出见为环状断离宫颈,周径4cm,宽1.5cm,厚0.5cm。胎膜已破,先露为顶位,于棘
