目的探讨硫酸镁(MgSO4)治疗对大鼠蛛网膜下腔出血(SAH)后急性脑血管痉挛(CVS)血中一氧化氮(NO)和内皮素-1(ET-1)水平的影响。方法将128只Wistar大鼠随机分为正常对照组、假手术组、单纯SAH组和SAH+MgSO4组。采用枕大池二次注血法制作大鼠SAH后急性CVS动物模型,分别于术前1h和术后1h、6h、24h采用显微图像分析仪测量各组基底动脉直径,用硝酸盐还原酶法测定各组血清NO水平,用放免法测定各组血浆ET-1水平。结果单纯SAH组在术后基底动脉直径缩小,SAH+MgSO4组在术后基底动脉直径大于单纯SAH组,但小于正常对照组和假手术组,差异均有统计学意义(P<0.05)。单纯SAH组术后血清NO水平较正常对照组和假手术组降低;血浆ET-1水平较正常对照组和假手术组升高,差异均有统计学意义(P<0.05)。SAH+MgSO4组术后血清NO水平较单纯SAH组升高,但低于正常对照组和假手术组;血浆ET-1水平较单纯SAH组降低,但高于正常对照组和假手术组,差异均有统计学意义(P<0.05)。结论血清NO水平和血浆ET-1水平的变化和动态平衡的破坏与SAH后急性CVS有关。MgSO4的治疗能够使血清NO、血浆ET-1水平恢复正常从而起到防治SAH后急性CVS的作用。
Objective To investigate the effects of magnesium sulfate on the levels of nitric oxide (NO) and endothelin-1 (ET-1) in acute cerebral vasospasm (CVS) after subarachnoid hemorrhage (SAH) in rats. Methods 128 Wistar rats were randomly divided into normal control group, sham operation group, simple SAH group and SAH + MgSO4 group. Acute CVS animal model of SAH was made by the secondary injection method of occipital large pool. The diameters of basilar artery in each group were measured by microscopic image analyzer at 1 hour before surgery and 1 hour, 6 hours and 24 hours after surgery respectively. Nitrate reductase The levels of serum NO in each group were determined, and the levels of plasma ET-1 in each group were determined by radioimmunoassay. Results The diameters of basilar arteries in SAH group decreased only after SAH + MgSO4 group. The diameter of basilar artery in SAH + MgSO4 group was higher than that in SAH group alone, but smaller than that in SAH group and sham operation group (P <0.05). Compared with normal control group and sham operation group, serum NO level in SAH group was significantly lower than that in normal control group and sham operation group (P <0.05). The level of serum NO in SAH + MgSO4 group was higher than that in SAH group, but lower than that in SAH group and sham operation group. The plasma ET-1 level was lower than SAH group, but higher than that in SAH group and sham operation group All were statistically significant (P <0.05). Conclusion The changes of serum NO level and plasma ET-1 level and the destruction of homeostasis are related to the acute CVS after SAH. MgSO4 treatment can make serum NO, plasma ET-1 levels returned to normal and thus play a role in the prevention and treatment of acute CVS after SAH.
