[Objective] To investigate the relationship between plasma endothelin (ET), homocysteine (Hcy), brain natriuretic peptide (BNP) and diabetic nephropathy. [Method] The levels of plasma ET, Hcy, BNP and urinary protein excretion (UAER) in 104 diabetic patients and 47 healthy controls were measured. [Results] The plasma levels of ET, Hcy and BNP in non-nephritic patients with DM were not significantly different from those in normal controls (P> 0.05). The plasma levels of ET and Hcy in patients with microalbuminuria and clinical albuminuria were significantly higher than those in normal controls The difference between the control group was statistically significant (P <0.05). Correlation analysis showed that there was a positive correlation between plasma ET, Hcy, BNP and urinary protein excretion rate (UAER) in patients with diabetic nephropathy in clinical albuminuria group. [Conclusion] The levels of plasma endothelin, homocysteine and brain natriuretic peptide are closely related to the occurrence and development of early diabetic nephropathy.
